Employee failed to use 3 point contact when exiting haul truck and stepped onto a rock on unpaved ground and turned ankle.

Employee sat down for his morning break, when he developed stroke symptoms. Another employee called 911 and the employee was transported to the local hospital. The employee was diagnosed with 100% blockage in his artery in the neck and is undergoing angioplasty surgery.

A piece of metal was being removed - Mis-communication. Employee was trying to get in position to help when a piece of metal was pushed striking employee on right hand causing break to right ring finger.

Employee was attempting to remove granule chute sock to prepare for car loading.He was reaching to unfasten the chute & when it came loose, he could not hold on to it and dropped it. He suffered strain to right shoulder. This became a recordable after a follow up physician visit prescribed a refill on his medication.

EE WAS DIGGING OUT A DRAINAGE DITCH IN THE BOTTOOM LEVEL AT THE QUARRY WITH THE CAT 235 SHOVEL WHEN HE NOTICED HYDRALIC FLUID SPRAYING THE EE WAS GOING TO SHUT THE ENGINE OFF WHEN THE FIRE ER UPTED THE EE BURNED THE BACK OF BOTH HANDS REACHING FOR THE FIRE EXTINGUISHER. THE EE & OTHER EE'S FTHAT RESPONDED PUT OUT THE FIRE WITH THE FIRE EXTINGUISHER NO LOST TIME
