Over-exertion in lifting objects

Employees were changing plant screens. While carrying the screens up the stairs employee strained their back. There were 3 employees carrying the screen. One on either side and employee was supporting the middle section. The employee at the front of the screen lost grip causing the screen to slip. Employee attempted to catch the slack and tweaked lower back in the process.

Employee and co-worker were changing rollers on a belt. When they were removing the old roller, it broke hitting employee's finger that was on the railing.

Over-exertion in lifting objects

EE was moving a large electric motor up a flight of stairs to a shaker at the main crusher and strained his lower back. The strain was enough for a doctor visit. The doctor prescribed muscle relaxants and 3 days of restrictive duty of no more than 10 pounds of lifting.

Fall to the walkway or working surface

While moving from water truck to welding truck, employee stepped on some loose metal pieces causing him to slip and fall to the ground, hitting his forehead.

Handing down screen to lower level storage area, screen caught glove of right hand and hung onto it forcing hand between screen & angle iron safety rail causing cut finger.

Struck against a moving object

The truck was watering pit haul road, driver was going too fast and could not stop the truck. Driver turned truck into road berm and truck rolled over, scrapped truck.

Caught in, under or between a moving and a stationary object

Miner was using old truck seat for a chair. He started to move the seat & as he set down the seat went down pinching off the tip of his finger. His finger was caught between the seat frame and the seat bottom. Cut tip off of middle finger on left hand.
