Working on Crusher and some metal flew up and lodged in Arm above glove

Employee was using the impact in right hand and got cut on the left middle finger. Employee received stitches.

Laying on the shaker. Lift a piece of belt above head and twisted wrong felt burning and pain down lower back and legs

Swinging sledge hammer to knock something off & missed pain in r shoulder. Swinging hammer and missed pulled arm. arm now is burning & sharp pain

Working on crusher a piece of debrie flew up and hit Paul in the mouth

EE stepped out of the truck the wrong way and hurt his lower part of his back.

Slope gave away under the excavator ee was operating. Excavator went over on its side ee was not hurt. Their was a laceration another ee pushed ee off the slope that broke off highwall. EE fell backward about 12ft where ee landed on a huge boulder in the middle of his back and other ee landed on top of ee. EE couldn't walk after that it broke L1 vertebra in the back.

While cutting a rubber hose, knife closed cutting employee's finger.

He was shoveling roots and putting them in the bobcat and hurt his back.

EE was loading blocks and some metal chipped off and got in his eye.

Bending over and taking the blocks out of the crusher.

HE WAS BACKING UP ON THE 821 LOADER. THE SIDE GLASS HUNG ON A TREE LIMB AND BROKE THE GLASS. A PIECE OF THE GLASS WENT IN HIS LEFT EYE.

HE WAS PULLING WIRES THROUGH THE LIGHT FIXTURES AND HIS HAND SLIPPED AND CAUGHT THE SIDE OF THE LIGHT FIXTURE

Employee was pulling on the motor and picking up some spray bars and hurt his arm.

EE WAS STANDING ON A LADDER, CLEANING CRUSHER CHUTE. CHUTE BROKE AND KNOCKED EE AGAINST HANDRAILS AND DOWN TO DECK.

WALKING ACROSS FLOOR IN KITCHEN AREA OF AGGREGATE OFFICE TRAILER, SHE STEPPED ON A SLICK SPOT ON THE FLOOR AND FELL ON HER WRIST.
