Employee was trying to remove a 100lbs rock off of a belt. Employee picked up the rock, swung to EE's left side smashing EE's finger into a metal railing. The rock smashed EE's pinky resulting in stitches.

EE was checking fuel levels and mis-stepped off of a ladder. EE did not completely fall, but EE did hit EE's knee on the rung.

Employee did not follow the 3 Point Rule, thereby, failing to comply with company policy on equipment procedure. He fell from the loader cat walk. Resulting injury was compression fraction to T7 vertebrae. Prognosis is 6 weeks in back brace.
