Miner had been mentioning back pain for approx. 2 months, when questioned; he stated that it was not injured at work. At the beginning of his shift on 4/23/12 he was in obvious pain and agreed to not work and see a doctor, again insisting his injury was not work related. On 1/8/13 we were notified by the insurance carrier that the injury is work related.

Miner awoke Monday 11/5/07 with right knee pain. He reported to work for his normal shift. Miner stated the pain was not work related and decided to go to his doctor. Miner left work at approx. 7:00 AM. The next day, after his Dr. visit, the miner notified Human Resources that the injury may be work related. He was then sent to the appropiate medical care.
