Employee was installing new bearing on tail pulley and put EE's hand in pinch point between bottom of bearing and conveyor frame. Employee suffered laceration to middle finger requiring six stitches.
Employee ALLEGES that the condensate line on the air conditioner mounted on a WA600 front end loader ""belw up"" in his face causing chemical burns to his lungs. Did not report the injury, did not come to work on 7-8-10 and called in sick. He admitted himself to the hospital on 7-9-10 in the evening. The mine operator was made aware of the employees hospitalization on 7-12-10.
Employee was standing on a platform at the feeder belt and was attempting to turn the tail pulley with a long bar. Another worker was on the ground behind him helping to turn the bar. When the employee placed his foot on the bar he stepped on the other workers hand. this startled the employee, he lost his balance, fell backwards, landing on some cobbles.
Employee was lifting hydraulic pump and strained lower back.
Employee was walking down stairs at the screening plant and strained left knee.
Miner was lying in the middle deck of the Tyler Screen when his hammer began to slide downhill. As he attempted to grab it he sat up and hit his forehead on a steel beam.
Employee was tightening bolts on a shaker screen hold-down. He slipped and fell backwards striking a handrail.
EE was prying on tail pulley guard of 3/8"" reject conveyor and pry bar slipped and struck EE on the lip. Area was wet from process water and rain.
WORKER WAS CUTTING CONVEYOR BELTING WITH KNIFE WHEN IT SLIPPED FROM SWEATY HANDS. THE KNIFE FLEW UP IN THE AIR AND STRUCK WORKERS LEFT LEG.
DRIVER HIT EDGE OF BERM CAUSING STEERING WHEEL TO SPIN. RIGHT THUMB GOT CAUGHT IN SPOKES, WHICH CAUSED WRIST SPRAIN.
WORKER WAS WALKING UNDER A CONVEYOR AS IT WAS BEGINING TO MOVE. A COBBLE ROLLED OFF THE BELT AND STRUCK WORKER ON THE HEAD. HE WAS WEARING A HARD HAT. COBBLE FELL ABOUT 4'.
