A fire at the Pyrite Street Location took place. A hydraulic steering hose burst while A 988 loader (company loader # 360) was in operation, splashing fluid on the motor causing a small fire in the engine compartment. The fire was extinguished with in 3-4 minutes. The Riverside County Fire Department was called as a preventative measure Incident # 111914.

Employee was trying to remove a plate from jaw crusher. He hit it with a hammer to loosen. At that time, a bracket broke and flew off. The come-along came loose and hit the employee in the face, right side, breaking a tooth.

While individual was removing spring compressing tool, the all-thread rod became disengaged from hex head holding nut. The spring compressing tool's spring holding plate struck the individual on the left side of his face. The individual sustained a laceration and abrasion to his left check area. No equipment damage occurred.

EE WAS PLACING A MOTOR ON ITS PLATFORM USING A BOOM CRANE. WHILE MOVING THE MOTOR A BOLT HUNG UP THE MOVE. THE BOLT CAME LOOSE AND THE MOTOR BOUNCED HITTING THE EE LT HAND THAT WAS UNDER THE M OTOR AT THAT POINT.
