Miner was on the deck of a screen, positioning the safety cage when cage came loose and hit the miner, who then sat on the deck and passed out, fell through the railing to the ground head first.

Employee did pre-inspection, started Volvo A35, drove to mud station where truck is loaded. When stepping out of truck, he stepped from the deck to the bank instead of using ladder off of equipment. Slipped and missed the bank and hit chest on the top of the bank.
