EMPLOYEE PULLING ELECTRICAL WIRES AND SLIPPED FROM LADDER, STRAINING RIGHT SHOULDER

TWO EMPLOYEES WORKING ON CONVEYOR BELT ROLLER. PRY BAR BEING USED TO HOLD UP ROLLER SLIPPED AND EMPLOYEE FINGER SMASHED BETWEEN ROLLER AND CONVEYOR SIDE.

EMPLOYEE WAS WORKING AROUND WET PLANT, IN MUDDY AND SLIPPERY CONDITIONS, STEPPED IN HOLE AND TWISTED RIGHT KNEE.

Employee was helping to install pump on wet plant and used conveyor leg as a ladder, upon coming down slipped and fell about 5 feet to ground, injuring left arm/wrist, and left leg.

Shaft used to secure belting slipped and struck employee's head. Employee fell as a result of the blow to his head and struck the back of his head on the floor. Employee suffered a laceration to his head and possible concussion.

Employee stepped into a hole and twisted his knee.

EE was walking down the side of a stockpile. He slipped and twisted his knee.

Employee was replacing the tailguard on a conveyor & strained his lower back.

Employee was lifting a piece of pipe and strained his lower back.

PULLY BELT SNAPPED AND BELT CONNECTOR HIT THE EMPLOYEE ON THE BACK OF HIS RIGHT HAND.

CHANGING BEARING IN EQUIPMENT, CO-WORKER ACCIDENTLY STRUCK HIS MOUTH WITH A HAMMER ON THE BACKSWING.
