Miner was clearing accumulated rock from conveyor when rock was dislodged and struck miner in the leg and ankle causing contusions and abrasion.

Over-exertion in pulling or pushing objects

Miner hyper-extended big toe while working on pipe. Miner was pushing pipe with foot when injury happened. Miner reported heat related injury on 6/30/15 and went to Urgent Care on 7/1/15. While there, doctor cleaned up infected big toe. Miner reported big toe injury to Supervisor on 7/5/15. Miner was admitted to hospital on 7/3/15.

Changing a screen on our McClosky, came down the ladder and stepped wrong and twisted his knee.

Miner was lubing screen. When he stepped away from screen, he stepped on cobble twisting left ankle. Miner reported incident on April 13, 2010, at 7:00 a.m. He left the site on 4/13/10 and went to Comp Clinic to get ankle checked out.
