while climbing on the ladder, lost footing and slipped off ladder, falling to the ground landing on ankle, and backside

a rock flew out of the plant and struck miner on the L side of the face, casing blunt eye trauma, orbital floor closed fracture. Laceration of eyelid. no Eye injury.

Miner drove bulldozer over uneven ground, EE felt a pinch in the buttock area. The compression caused bruised tailbone.

L shoulder sternoclavicular sprain, R shoulder strain. Worker lifting tool above shoulder, sustained pinch nerve/strain/pain

While lifting an item, felt pain in groin area. Swelling 3 days later. Reported injury on 2/22/22.

while removing a gear box, left finger was pinched, causing a contusion, laceration, and fracture of proximal phalanx.

Miner was grinding metal, debris flew into right eye, resulting in irritation

While on ladder, Miner was using a drill to attach screws to rubber patch, ladder slipped on wet floor and miner fell on L wrist.

Audiometric test on 3/25 indicating a recordable hearing shift, received the report on 4/5/2019, shared findings with worker on 4/8. Retest is scheduled on 4/16/2019.

while moving a motor onto the motor plate, the plate slipped and pinched Miner's right hand, causing laceration and minor fracture

EE failed to chock tire or de-energize tractor prior to attempting repairs. Tractor rolled forward, tire rolled over foot of EE

EE slipped hitting his head on a metal guard.
