EE opened the electrical panel door and was shocked. EE then fell back hitting a vice attached to a work bench dislocating EE's shoulder. EE had burns to EE's right middle & ring finger & dislocated EE's left shoulder.

Changing bucket teeth on a 922 Loader when a piece of tooth broke off while hitting it with a hammer. The piece of tooth penetrated the upper left leg of the employee which needed to be removed and stitch up

The employee was working at Unit #2 6th floor 2nd stage Cyclone passing material into the man way. While passing a piece of plywood into the man way he wedged his finger between the man way and the piece of plywood.
