The Miner failed to follow the lock out/tag out procedure by de-energizing the conveyor and locking it out, and was accidentally shocked while attempting to plug in the #4 S.O Cord to the 3/4 Washed conveyor as the result. EE was discharged from the Hospital within the day. EE suffers from a sore neck and shoulders.

Two miners rigged conveyor to move. Upon lifting, the rigging became unhooked. While attempting to lower it the conveyor flopped over. The miner on the ground was standing to close and as it rolled over one of the adjusting screws caught him in the lower leg. The miner was taken, by ambulance, to closest hospital. Result was a broken lower right leg.
