Employee stacks stone on pallets. Called me morning of 07/23/2020 said arm and shoulder was hurting, took the day off. Worked on 07/24/2020 and 07/27/2020, called me this morning 07/28/2020, Arm and shoulder are hurting, sent them for evaluation. Doctor report indicates a trapezius strain, L. shoulder stain and L. elbow strain.

Employee reported pain in lower back or hip on 6/3/15, said he had minor pain last Friday 5/29/15, but is now getting worse. Does not seem to know what caused the pain, thought maybe a twist. Doctor has him on restricted weight limit and is currently unable to perform job.

Employee was lifting stone, he reached forward to place stone on a pallet and had chest pain. Employee was sent to CCOM for evaluation. Doctor indicated the sternum muscles were inflamed. Doctor will schedule employee for physical therapy. Employee will return to work when he is able to perform stone palletizing tasks.
Employee was hauling stone from a stock pile. A semi-truck & trailer was in the area being loaded by another operator. EE said he was distracted by other activity going on in the staging area and inadvertently veered into the front end of the truck. No one was injured, but there was damage to the truck.

Worker picked up a piece of stone. The stone broke in his hands and a piece flipped and landed on his foot.

Employee states he experienced back pain when lifting stone on Nov. 4th, thought he was OK, but then on Nov. 5th he had more pain when wrapping a pallet. Rested during the weekend and said he felt good in the Morn. on Nov. 8th, after working for a while the pain came back, he reported to manager. Employee went to provider Nov. 9th AM. Doctor told employee he has a back strain.
