Quarry worker, was cutting rock with the grinder, wiped EE's safety glasses with EE's glove and took them off, and wiped EE's eye while getting the silt off EE's face with EE's glove, which EE thought was clean and pushed debris into EE's eye.
Maintenance worker was trying to fix a water pump, and tried to remove the Allen screw and instead moved the pulley and sliced left palm.

On loading a stone onto the stone cutter rollers, stone slab caught the corner of the table and tilted causing reaction to tilt the rock back onto the rollers, and in the midst of trying to maneuver the rock back onto the rollers, the slab caught the worker's middle finger resulting in an injury.

EE was unloading rock with the forklift. EE tried to push the rock from the forks by hand and caused their finger to get caught which caused a laceration to EE's left index finger.
On 03/24/2021 employee let me know that they had been having some pain in right wrist. Employee said that the pain started a week prior and they got a wrist brace for it, and the pain still continued to get worse. Employee splits stone and uses a sledge hammer with a chisel to do so.

EE was chiseling stone and got a piece of metal from EE's chisel in EE's leg.
EE was using the grinder when EE felt the grinder started to feel like it was becoming loose. The kill switch was located directly near the grinding wheel and guard. EE was slowly moving the grinder down towards the ground so the wheel wouldn't hit EE. As EE did the wheel came off and so did the guard.

Employee was moving some rocks around and felt a strain in the back. As the week progressed, EE reported that the pain started to get worse and EE could feel it in the leg.

EE reported to us on 11/21/2017 that EE had been feeling some abdominal pain. EE had a hernia two years ago which required surgery. I made an appointment for the EE right after Thanksgiving break.
EE was grinding on a bolt cutting, the cutting wheel disintegrated and a small piece of metal got in EE's eye. EE was wearing safety glasses at the time of the accident.
EE was walking back to loader which was parked near a 5ft ledge. As EE was climbing in EE placed hand on the loader to get in, slipped and lost balance. EE fell backwards down the ledge.
Employee was walking down an area where rock had been quarried and ground was unstable when they slipped down and fell back on their hands. They injured their right hand due to trying to catch their fall.
Employee was moving slabs from on spot to another when employee got employee's finger caught in between two and split it open.
There was a low drawer open in the office and employee ran into it injuring employee's shin. Employee had some major swelling and bruises.

EE has been an employee here with the company for about 6 years. EE started feeling a pain in lower stomach area and reported it to supervisor, which supervisor gave ee a back support for to help. EE reported a week later that the pain was getting worse.
EE reported on 10/03/2016 that they felt some pain in abdominal area. EE said they felt a bump in May but it did not bother them until now. EE had a hernia the previous year and thinks they may have another one.
EE was lifting pallets of large stone on to our rollers where are cutters are and said ee back was hurting really bad. EE then reported to one of the supervisors who then reported it to me.
EE was helping assist a maintenance employee in fixing EE's cutter when a piece of metal wire sprung up and struck EE in right arm. EE pulled the wire out but felt there was a piece still stuck in EE's arm.
EE had felt a little lump near belly button area and wasn't feeling any pain at the time. EE started feeling a soreness where the lump was on 07/24/2016 and thought it may go away, it had only gotten worse so EE reported it on 07/26/2016.
EE was chiseling a piece of stone with a hammer when EE hit right hand with the hammer. EE felt EE would be okay the day of injury and then reported the pain wasn't going away a week later.

Employee was off bearing and pulled a rock closer towards him, didn't realize the rock was heavier then expected and it fell on his foot. He was wearing steel toed boot but he feels some pain in his foot.
Employee had came into my office on 8/27/2015 explaining to me he had injured his finger the week before and the swelling had not went away. He was lifting up stones and placing them down, he had got his finger caught under one as he was placing it down causing an injury to his right pinky and ring finger.
Employee was lifting a heavy rock and turned to put the rock down when he felt a sharp pain down his back/side area.
EE mentioned to his quarry supervisor that he was feeling some pain in his back and had been since the previous Friday 7/31/2015, but didn't notify anyone until 8/4/2015. He told the supervisor the pain was getting worse so he needed to make an appointment.
Employee had reported to his supervisor that he had been feeling a pain in his stomach and wasn't sure where it came from. He thought he may have a hernia or strained a muscle somewhere in his abdomen. I set him an appt for 05/27/2015 to be seen.
EE reported to me on 4/15/15 stating his arm/shoulder had been hurting him and he was going to get seen at his physician's office, however he has said it had been hurting for a couple of weeks and wasn't sure what the cause was. The physician's suggested him to make a workers comp claim because his job consists of repetitively moving his right arm up and down.
Employee reported to me on Friday the 13th that he had got a metal shaving in his lip and he tried to get it out himself, but it didn't come out so he asked to make an appt. I scheduled him on Monday the 9th and he was seen on Tuesday the 10th.

Employee is a stone polisher/cutter, he started to develop a bump on his wrist, he was slightly worried so we sent him to get it checked out. He has a cyst that the doctor said would go away or they can remove it for him. He was told to see a follow up physician.

Employee was placing stone onto conveyor system for thin cut saw when the piece split into two pieces and one of pieces fell on top of his right foot between the steel toecap and the ankle. Employee was sent for X-rays, which were negative and told to return to work on Wednesday.

His supervisor saw him asked him if he was hurt he said that he was, but he thought it would get better over the weekend so he didn't wanted to go to the doctor, but on Monday he told his supervisor that his ribs still hurt and he wanted to go and see the doctor.

EE was pulling some wire through a conduit with a pair of needle nose pliers. The conduit was situated at about eye level. The pliers slipped off of the wire and struck the EE below his left eye. He was required to have stitches.
The employee reported a lump on his left hand possible due to repetitive actions. When returning for a follow up appointment on 5/18/11 he was placed on restrictions, prescribed vicodin and surgery was scheduled.

Employee was off bearing stone from a stone cutter. There are two forks that come up from the table to ""catch"" the stone. The stone jumped and the employee caught his little finger between the stone and one of the forks. He received 7 stitches and was prescribed hydrocodone.
The ee was climbing out of the excavator and slipped on snow from the tracks and landed with his left hand on the rail on the left side of the track. He was prescribed vicodin.
EE started work in the morning feeling fine. His lower back suddenly began to hurt on the right side. EE cannot pin point the activity that may have caused this. EE was prescribed Motrin, vicodin and medrol and put on temp. restrictions.

The doctor stated that there was not a pinpointable accident, but a cyst possibly caused by overuse. No restrictions were imposed but the employee was referred to an orthopedic specialist. TOOK WHEEL OFF TRUCK. LEANED WHEEL AGAINST BLOCK WALL. WHEEL TIPPED BACK OVER ONTO HIS LEG.

EE WAS ATTEMPTING TO PRY DOWN A TOOTH THAT WAS STUCK ON STONE CUTTER. WHEN 3' IRON BAR HE WAS USING SUDDENLY CAME LOOSE, SMASHING HIS FINGER BETWEEN THE BAR AND THE METAL OFF BEARING LEDGE.
HE WAS DISMOUNTING A FRONT-END LOADER AND STEPPED ON A ROUND-SHAPED ROCK WHICH CAUSED HIS KNEE TO TWIST.

WHILE EE WAS SPLITTING SLABS WITH ANOTHER EE, EITHER THE WEDGE OR SLEDGE HAMMER THAT HE WAS UYSING HAD A SMALL PIECE FLY OFF AFTER STRIKING THE WEDGE WITH THE SLEDGE. THE PIECE PUNCTURE THE UP ER INSIDE OF HIS LEFT THIGH. A FOREIGN OBJECT WAS NOT REMOVED OR FOUND.
EMPLOYEE INSERTED FINGER UNDER A 12"" X 12"" STONE IN ORDER TO MOVE THE STONE INTO POSITION ALONG THE ROLLERS OF A STONE CUTTER. WHEN HE MOVED THE CONTROL STICK TO LOWER THE HYDRAULIC CUTTER ONT O THE STONE TO SHEAR IT, THE ROLLERS LIFTED CAUSING HIS RIGHT MIDDLE FINGER TO BE SMASHED BETWEEN THE ROLLERS AND STONE.
EE WAS ATTEMPTING TO WALK THE STONE OVER TO A PALLET WHEN HE FELT A PULL IN THE RIGHT SIDE OF HIS BACK.
WHILE EE WAS SPLITTING STONE, A PIECE FROM THE WEDGE EMBEDDED ITSELF INTO HIS RIGHT THIGH.
EE WAS DISMOUNTING FROM A TRUCK WHEN HIS FOOT SLIPPED FROM THE STEP. EE GRABBED ONTO BAR ON SIDEOF TRUCK AND FELT A PULL IN HIS ARM.

EE WAS REMOVING STONE FROM CUTTER WHEN HIS HAND WAS SMASHED BETWEEN THE BAR & THE STOEN
EE WAS CHISELING STONE AND HIS HAMMER MISSED THECHISEL, EE INSTEAD STRUCK THE INSIDE OF HIS LEFTKNEE.
EE WAS MOVING (WALKING) A LARGE STONE TO PLACE IT ON A NEARBY PALLET. WHILE HE WAS MOVING IT HE TWISTED HIS BACK AWKWARDLY AND FELT A TWEAK IN HIS BACK. AFTER PLACING THE STONE ONTO THE PALLET HE FELT A PULLING SENSATION IN HIS BACK. SHORTLY AFTERWARDS, THE EE DESCRIBES THAT HE WAS HAVING ""MUSCLE SPASMS"" AT THE MIDDLE TO RIGHT SIDE OF HIS MID-BACK.
