Quarry worker was cutting flagstone, piece of chisel flew off and hit their left forearm causing a contusion.

Quarry worker was splitting at stone, and a layer of the rock that EE was splitting broke off and fell on EE's left foot.

Injury occured on 7/18/24, employee had let the company know it was work related injury on 7/25/24. Warm sunny day, cutting rocks on cutter machine, rock split, and broke into smaller pieces, and one bigger piece struck worker on the right forearm causing pain in right elbow.

Employee was clearing the cutter and tossed rocks to the side of the cutter. EE cut right hand while tossing the rocks.

Employee was off bearing on a cutter, machine crushed right wrist.

Employee smashed right hand on the palm under EE's knuckles moving a rock onto a pallet.

Employee had stated was trying to top cut a rock with an 8lb sledgehammer and hit left-hand middle finger.

Employee was operating the Saw shop Corner machine. While readjusting the rock, the table slid causing a right-hand laceration to the top of the right hand getting caught between the rock and the saw.

On loading a stone onto the stone cutter rollers, stone slab caught the corner of the table and tilted causing reaction to tilt the rock back onto the rollers, and in the midst of trying to maneuver the rock back onto the rollers, the slab caught the worker's middle finger resulting in an injury.

EE was unloading rock with the forklift. EE tried to push the rock from the forks by hand and caused their finger to get caught which caused a laceration to EE's left index finger.

On 03/24/2021 employee let me know that they had been having some pain in right wrist. Employee said that the pain started a week prior and they got a wrist brace for it, and the pain still continued to get worse. Employee splits stone and uses a sledge hammer with a chisel to do so.

EE was chiseling stone and got a piece of metal from EE's chisel in EE's leg.

EE was using the grinder when EE felt the grinder started to feel like it was becoming loose. The kill switch was located directly near the grinding wheel and guard. EE was slowly moving the grinder down towards the ground so the wheel wouldn't hit EE. As EE did the wheel came off and so did the guard.

Employee was moving some rocks around and felt a strain in the back. As the week progressed, EE reported that the pain started to get worse and EE could feel it in the leg.

EE was hammering a part trying to get a bearing off of a pulley, when a little piece of metal came off of the punch and hit ee in the left side of the chest.

EE was chiseling a piece of rock when a metal piece from the chisel broke off into EE's leg.

EE was splitting rock when a metal piece shot up and hit EE in EE's chest.

EE was walking in the saw building when EE was called over to the maintenance department, when EE was walking over EE nicked the top of shoulder on our 10' saw blade.

EE was trying to help coworker adjust the cutter when ee finger was caught, causing ee left pinky to get stuck and possibly break. The nail broke and ee was wearing gloves.

EE was working in our yard stacking some stone on a pallet when a loose piece of stone fell down on foot.

EE reported to us on 11/21/2017 that EE had been feeling some abdominal pain. EE had a hernia two years ago which required surgery. I made an appointment for the EE right after Thanksgiving break.

EE was grinding on a bolt cutting, the cutting wheel disintegrated and a small piece of metal got in EE's eye. EE was wearing safety glasses at the time of the accident.

EE was walking back to loader which was parked near a 5ft ledge. As EE was climbing in EE placed hand on the loader to get in, slipped and lost balance. EE fell backwards down the ledge.

Employee was walking down an area where rock had been quarried and ground was unstable when they slipped down and fell back on their hands. They injured their right hand due to trying to catch their fall.

Employee was moving slabs from on spot to another when employee got employee's finger caught in between two and split it open.

There was a low drawer open in the office and employee ran into it injuring employee's shin. Employee had some major swelling and bruises.

Employee was operating a Cat 963 Track Loader and backed into a 4 ft. hole. The loader rolled over in the hole, there was no injury or damage to the machine.

EE was in the quarry walking to fix a cutter and slipped on some ice. When EE fell, EE landed on hands to try and catch self and ended up injuring left shoulder/pectoral.

EE has been an employee here with the company for about 6 years. EE started feeling a pain in lower stomach area and reported it to supervisor, which supervisor gave ee a back support for to help. EE reported a week later that the pain was getting worse.

EE reported on 10/03/2016 that they felt some pain in abdominal area. EE said they felt a bump in May but it did not bother them until now. EE had a hernia the previous year and thinks they may have another one.

EE was lifting pallets of large stone on to our rollers where are cutters are and said ee back was hurting really bad. EE then reported to one of the supervisors who then reported it to me.

EE was helping assist a maintenance employee in fixing EE's cutter when a piece of metal wire sprung up and struck EE in right arm. EE pulled the wire out but felt there was a piece still stuck in EE's arm.

EE had felt a little lump near belly button area and wasn't feeling any pain at the time. EE started feeling a soreness where the lump was on 07/24/2016 and thought it may go away, it had only gotten worse so EE reported it on 07/26/2016.

EE was lifting a rock when it fell on EE's foot, EE was wearing steel toe boots but the rock fell on the upper part of EE's foot where there was no protection from it.

EE was chiseling a piece of stone with a hammer when EE hit right hand with the hammer. EE felt EE would be okay the day of injury and then reported the pain wasn't going away a week later.

Miner was using a metal chisel on a piece of stone when a metal piece came off and went into EE's left thigh.

Employee was off bearing and pulled a rock closer towards him, didn't realize the rock was heavier then expected and it fell on his foot. He was wearing steel toed boot but he feels some pain in his foot.

Employee had came into my office on 8/27/2015 explaining to me he had injured his finger the week before and the swelling had not went away. He was lifting up stones and placing them down, he had got his finger caught under one as he was placing it down causing an injury to his right pinky and ring finger.

Employee was lifting a heavy rock and turned to put the rock down when he felt a sharp pain down his back/side area.

EE mentioned to his quarry supervisor that he was feeling some pain in his back and had been since the previous Friday 7/31/2015, but didn't notify anyone until 8/4/2015. He told the supervisor the pain was getting worse so he needed to make an appointment.

Employee had reported to his supervisor that he had been feeling a pain in his stomach and wasn't sure where it came from. He thought he may have a hernia or strained a muscle somewhere in his abdomen. I set him an appt for 05/27/2015 to be seen.

EE reported to me on 4/15/15 stating his arm/shoulder had been hurting him and he was going to get seen at his physician's office, however he has said it had been hurting for a couple of weeks and wasn't sure what the cause was. The physician's suggested him to make a workers comp claim because his job consists of repetitively moving his right arm up and down.

Employee reported to me on Friday the 13th that he had got a metal shaving in his lip and he tried to get it out himself, but it didn't come out so he asked to make an appt. I scheduled him on Monday the 9th and he was seen on Tuesday the 10th.

Employee is a stone polisher/cutter, he started to develop a bump on his wrist, he was slightly worried so we sent him to get it checked out. He has a cyst that the doctor said would go away or they can remove it for him. He was told to see a follow up physician.

EE was moving some rocks and felt a strong pain in his back, he continued to work and the pain did not go away so he went to the hospital.

Employee was lifting up a stone (10lbs) when he felt a slight pain in his back, he said it was getting worse so we scheduled him an appointment to get checked out.

EE was trying to push a heavy rock when the rock rolled back and smashed his hand. Another worker took him to the emergency room where they concluded he had an open fracture on his 5th finger.

Employee was cutting stone and turned too suddenly and strained a muscle in his abdomen. Dr said it could possibly be a hernia.

EE had went to the emergency room due to something in his eye. He found out he had a small metal piece in his eye that was removed, then cleaned.

Employee was chiseling a stone when a metal piece from the chisel shot him in the forehead. He was then taken to the emergency room to get the metal piece taken out. He had his laceration closed with Steri-Strip.

Employee was lifting rock when he had injured his back. He is not working and is required to do physical therapy for his Lumbosacral back strain.

EE was chiseling a rock when the rock chiseler was shedding pieces of metal in which one punctured EE chest, another EE then came to help EE and pulled the shard of metal from his chest. EE was then sent to the hospital.

Employee was shutting the gate and as he was shutting it he stepped back and down with his left foot on a rock and slightly twisted his ankle causing him to hurt his Achilles tendon.

Employee was carrying a rock and stepped back onto unstable ground and his leg went out, causing him to hurt his knee. This causing him pain in his leg and knee.

Employee was using a rock chiseler and stepped down from chiseling the rock and he stepped down on something that made him twist his leg.

Employee was chiseling a rock and a piece of rock got into his eye. He decided to wait and see if it would go away, then finally decided to get medical attention on 04/11/2014.

Miner was rolling a rock towards a scrap pile when he picked it up to place in the pile he felt a pain in his right abdominal area. Pain was a 3 on a scale where 1 is the lowest. He finished his work shift thinking that rest that evening would take care of problem, but still had pain when he returned to work the next day.

Worker was splitting rock with a hammer and chisel when he heard a popping noise and felt pain in his wrist. He finished his shift and then performed other duties the next 2 work days. When he attempted to split rock on the 3rd work day he had pain in his wrist.

Employee was placing stone onto conveyor system for thin cut saw when the piece split into two pieces and one of pieces fell on top of his right foot between the steel toecap and the ankle. Employee was sent for X-rays, which were negative and told to return to work on Wednesday.

Worker was holding a rock on cutter #46, when the stone was snapped his foot slipped and he felt a pain in his ribcage. His supervisor saw him asked him if he was hurt he said that he was, but he thought it would get better over the weekend so he didn't wanted to go to the doctor, but on Monday he told his supervisor that his ribs still hurt and he wanted to go and see the doctor.

The employee was lifting a rock and felt pain in his lower back. He was put on temporary restrictions and pain medication.

The employee was getting off the forklift, his right leg struck the ground and as he bent forward he felt a sharp pain in his back. He was put on restrictive duty.

Employee was working in our shipping yard, preparing a pallet of material for shipment. Employee slipped while wrapping the pallet and hurt his back. His pain increased after a short period of time and he was then sent to the local medical facility.

The employee was off bearing a piece of stone when a chunk broke off and hit his foot, causing it to fracture.

The employee was placing stone onto a pallet when his thumb was smashed between two pieces of stone. He was taking to a clinic where he received sutures and prescriptions.

The employee was polishing the side of a stone when he felt a piece of stone hit his eye. He did not report it immediately hoping it would get better. It did not. He was diagnosed with a corneal abrasion and given a prescription.

EE was splitting stone with a 10 pound hammer and chisel. Over a period of time, he felt pain in his right shoulder. He was prescribed Naproxen and placed on a ten (10) pound lifting restriction.

EE was pulling some wire through a conduit with a pair of needle nose pliers. The conduit was situated at about eye level. The pliers slipped off of the wire and struck the EE below his left eye. He was required to have stitches.

The employee reported a lump on his left hand possible due to repetitive actions. When returning for a follow up appointment on 5/18/11 he was placed on restrictions, prescribed vicodin and surgery was scheduled.

Employee was putting scrap stone into hopper. When his left arm extended he felt a pull in his elbow. He was prescribed Ibuprofen and physical therapy. Reported per Help Desk's advice

Employee was off bearing stone from a stone cutter. There are two forks that come up from the table to "catch" the stone. The stone jumped and the employee caught his little finger between the stone and one of the forks. He received 7 stitches and was prescribed hydrocodone.

The ee was climbing out of the excavator and slipped on snow from the tracks and landed with his left hand on the rail on the left side of the track. He was prescribed vicodin.

EE started work in the morning feeling fine. His lower back suddenly began to hurt on the right side. EE cannot pin point the activity that may have caused this. EE was prescribed Motrin, vicodin and medrol and put on temp. restrictions.

The employee noticed a bump on his left hand/wrist. It had slight pain accompanying the growing size. The doctor stated that there was not a pinpointable accident, but a cyst possibly caused by overuse. No restrictions were imposed but the employee was referred to an orthopedic specialist. The cyst was drained on 11/9/09.

The EE was carrying a rock to a pallet and slipped. The rock landed on the EE's left foot. The EE was prescribed ibuprofen and put on modified duty.

The employee was moving a rock with the assistance of another ee and felt pain in his back. He was put on temporary lifting restriction and prescribed vicodin.

Employee was using a chisel to split a rock when a piece flew into his eye. He was sent to a specialist to remove the imbedded object.

Employee was drilling holes for blasting when his foot slipped causing him to fall. He was prescribed ibuprofen and put on temporary lifting restrictions.

Employee states that while he was adjusting the saw table his foot slipped on the wet floor which may also have had some oil on it. His leg stretched out and he felt a slight pull in his groin. He was prescribed naproxyn and put on temporary restrictions.

Employee felt something in his eye while cutting stone on Monday. He was prescribed eye drops.

The employee was adjusting a large rock with a crowbar to cut it and strained his muscles. He was prescribed Vicodin, Motrin and Flexeril.

Employee was off bearing strips of stone when a stone fell off the cutter and landed on top of his right foot, just above the steel toe. Employee was placed on temporary restrictions.

The employee was moving a rock on a cutter table, when he pushed the rock, the rock stopped abruptly and he hit his chin on the rock. He recieved three stitches.

Employee was picking up a 200 lb roller with a forklift. When he put the roller down, he pinched his finger between the roller and the bracket. He is currently wearing a finger splint.

Employee states he strained his back on Wednesday, while off bearing. He was prescribed medication and placed on temporary lifting restictions.

Employee experienced groin pain after lifting some large rocks. He was placed on temporary lifting restrictions.

The employee was splitting stone when he resituated his foot on some gravel. The gravel caused him to slide twisting his ankle. He has been placed on Ibuprofen and using an ankle brace.

Employee was polishing stone when the polisher jerked causing his wrist to twist. An hour later a bump appeared on his wrist accompanied by pain. Employee complains of pain in his wrist and arm. Amendment: Put back on restricted duty on 9/25/08.

Employee came in the morning of 8/27/08 reporting of pain in his left shoulder. He stated that he had injured it the day beofre while polishing, He was given percocet and was placed on temporary restrictions.

The employee was drilling holes with a hammer drill when the drill bit caught and caused him to twist his back. He was placed on temporary lifting restrictions and vicodin.

Employee was stacking stone. While carrying a piece of stone he stepped on a rock and tripped. He twisted his leg, fracturing his tibia.

The employee cannot pin point a particular instance. The employee just reported pain in his shins. He was placed on temporary restrictions.

EE was moving a stone and felt pain in his chest on the left side. EE was put on temporary restrictions.

Employee was polishing stone and reported pain in his right shoulder.

The employee states it happened over time. The EE stated that he did not report his injury earlier because he believed it to have been caused by the new constant use of a once not used muscle. He was prescribed naproxen, vicodin and placed on temporary lifting restrictions.

EE was stacking rock onto a pallet, when the rock slipped out of his hand and caught his left middle finger, smashing it. Finger was splinted and EE placed on temporary restictions.

EE was stacking stone onto a pallet, when his foot slipped and his head went down and struck the rock. He received stitches above his eyebrow.

Employee reported on 11/12/2007 that he twisted his knee while off bearing on 11/9/2007. He wanted to wait and see if it got any better. Employee reported on 11/20/2007 that his knee was still hurting. He was placed on temporary lifting restrictions and was prescribed naproxen and vicodin.

Employee was off bearing and walking a large rock over to a pallet. He went to set the rock down and felt a pull on the left side of his lower back. He was prescribed ibuprofen, flexeril and put on temporary lifting restrictions.

Employee was breaking a stone on stone on the side of the almost full metal hopper when his finger came between the rock he was breaking and the rocks already in the hopper, smashing his finger. He was placed on temporary lifting restrictions and naproxen.

Employee noticed an abnormality in his abdomen. He is unable to point to specific mechanism of injury or date. He was given lifting restrictions.

EE was positioning a rock to be sawn. His left hand was holding the rock & template & his right hand was on the controls of the 24 inch prodigy saw. He then inadvertently pushed the wrong buttons & dropped the saw which was not in rotation on his left middle finger. He received stitches & was placed on temporary lifting restrictions.

Employee was lying on his back under a piece of machinery attempting to perform repairs. Employee states he was working for three hours and the repairs entailed a lot of reaching, bending and twisting. He was placed on modified duty.

Employee was stacking flagstone when he stepped on a rock and his foot slipped off. As he did so, he twisted his knee. Prescribed Vicodin and placed on modified duty.

He was repairing a stone cutting machine. He was trying to remove a hydraulic fitting with a wrench and injured his right arm/shoulder. He was perscribed naproxen and put on modified duty.

Employee was removing a bolt from a saw with a wrench. The wrench then slipped and he hit his right elbow. He was prescribed Naproxen and placed on modified duty.

He was moving 8 inch pipe when he felt a pull in his stomach. He was placed on modified duty and scheduled for surgery.

Employee complained of pain to his right shoulder. Placed on a 20 pound lifting restriction and prescribed Naproxen.

A maintenance employee was picking up a sheet of corrugated steel to replace some roofing when he caught his hand between the sheet and the steel sheet on the roof. Cut his finger and received stitches.

Employee reported to management on 4/18/07 that while lifting rock on 3/27/07 he felt a pull in his lower back. He states he did not report it immediately as he felt the pain might go away. It did not. Prescribed Naproxen and placed on lifting restrictions.

EE was stacking stone onto a pallet and stated he "twisted wrong". Placed on lifting restrictions and prescribed Naproxen and Vicodin.

Employee was repairing the wiring on a forklift. While stripping the wire he slipped and cut himself with the utility knife.

Employee was stacking rock onto a pallet. When he picked up a stone he twisted toward the pallet and felt a sharp pain in his lower back.

Employee was performing maintenance work on a truck. While reaching around to remove some electric wires, he felt a pull in his back. Prescribed ibuprofen, cyclobenzaprine and placed on lifting restrictions.

Employee was moving stone on a stone cutting machine when he felt a pull in his lower back. Placed on lifting restrictions and prescribed naproxen and vicodin.

Employee was splitting stone with a hammer and chisel. A piece of rock chipped off and struck him on the upper lip.

Employee was stacking rock on to pallet. A stone fell off of the pallet and struck him on the foot above the steel toe portion of his boot. Prescribed Naproxen and Vicodin and given lifting restrictions.

EE complained of pain in both of her thumbs and hand. Prescribed Ibuprofen.

Employee was moving stone on a stone cutting machine when he felt a pull in his lower back. Placed on lifting restrictions and prescribed Flexoril.

Employee was measuring stone when he stepped on a rock and slipped. He put out his right arm to catch himself. He did not fall to the ground. He complains of pain to his lower back and right elbow. Placed on lifting restrictions.

Employee slipped on some ice. He fell to the ground and struck his knee on a rock buried under the snow. Prescribed Naproxen and put on lifting and walking restrictions.

The employee was splitting rock with a hammer and chisel. While bent down, he felt a pull in his lower back. Prescribed Percocet and Flexorel.

Employee noticed pain at the base of his left index finger. A lump has also formed which has become increasingly larger over time. Prescribed 500 mg of Naproxen.

Employee was stacking rock onto a pallet when he felt a pull in his lower back.

Employee was stacking rock onto a pallet when he caught his left thumb between two pieces of rock. He received stitches and prescribed Keflex.

Employee was polishing rock with a rock polisher. His left side started to hurt. Prescribed Naproxyn 500 mg.

Employee was carrying stone, stepped on a rock and twisted his ankle. Prescribed Naproxen.

Employee was moving rock along rollers on a stonecutter. He caught his hand between the rock and the rollers.

Employee was stacking stone onto a pallet when he felt a pull in his lower back.

Employee was unloading stone off of the stone cutter when the rock he was holding snapped back and bent his hand backwards. He was prescribed Naproxin and given 10 pound lifting restriction.

EE was working and noticed a pain in his ribs. He does not remember any specific event or trauma leading to the pain. He was prescribed Naproxyn and given a 10 pound lifting restriction.

EE was stacking stone onto a pallet. He dropped the piece he was stacking onto his right hand. Suffered a laceration to the right index finger. Placed on a five pound lifting restriction.

Employee was stacking stone onto a pallet. He was backing up and tripped over another pallet. He fell and the stone he was carrying landed on his finger.

EMPLOYEE WAS CLEANING THE OVEN IN THE STAFF ROOM. SHE STATES HER ARM AND SHOULDERS BEGAN TO ACHE. SINCE THAT TIME, SHE STATES SHE HAS FELT VERY WEAK AND UNABLE TO KEEP HER ARMS LIFTED FOR AN EXTENDED PERIOD OF TIME. SHE WAS UNABLE TO EXPLAIN WHY IT TOOK A WEEK AND A HALF TO REPORT IT.

EMPLOYEE WAS STACKING STONE ONTO PALLET. HE DROPPED THE ROCK AND IT LANDED ON THE TOP OF HIS RIGHT FOOT.

EMPLOYEE WAS SPLITTING STONE WITH A HAMMER & CHISEL. HE FELT PAIN IN HIS MIDDLE FINGER OF HIS RT. HAND. THINKING THE PAIN WOULD GO AWAY, HE DID NOT IMMEDIATELY REPORT IT. A HARD BALL FORMED UNDERNEATH THE SKIN. PRESCRIBED 800 MG. OF IBUPROFIN. RELEASED TO FULL DUTY BUT REFERRED TO AN ORTHOPEDIC SPECIALIST FOR POSSIBLE SURGERY.

EMPLOYEE WAS CARRYING BUCKETS AS SHE WAS EMPTYING WATER OUT OF OUR MAN MADE POND. AS SHE WAS BENDING OVER SHE FELT A PAIN IN HER LEFT SIDE. SHE RESTED IT FOR SEVERAL DAYS THINKING THE PAIN WOULD GO AWAY. IT DID NOT. PRESCRIBED NAPROXIN.

EMPLOYEE WAS MOVING STONE WITH A PRY BAR. THE STONE MOVED AND HE SLIPPED AND FELL ON HIS BUTT. PRESCRIBED NAPROXIN AND FLEXORIL.

EE was removing waste stone off of the stone cutter and throwing it in a pile. He felt a pull in his side. He did not immediately report the injury. It became progressively worse.

Employee was splitting stone with a hammer and chisel. He struck his thumb with the hammer. He received stitches for a laceration to his left thumb.

Employee was stacking stone off of a stone cutting machine. A piece of stone fell off of the stone cutter and struck him on the right leg above the kneecap. Prescribed Naproxin.

EE WAS HELPING ANOTHER EE MOVE A FREEZER. HE COMPLAINED OF LOWER BACK PAIN LATER. PRESCRIBED FLEXORIL & NAPROXIN AND PLACED ON RESTRICTED DUTY.

EMPLOYEE WAS BENDING DOWN TO GREASE SOME EQUIPMENT. HE FELT A PAIN IN HIS RIGHT KNEE. KNEE DRAINED OF FLUID AND PRESCRIBED NAPROXIN.

Employee was breaking stone with a hammer and chisel. A piece of rock flew off and under the rim of his glasses landing in his eye. Eye cleaned and employee prescribed acular and polysporin.

EMPLOYEE WAS HOLDING A CHISEL WHILE ANOTHER EMPLOYEE HAMMERED. THINKING THEY WERE DONE, HE WENT TO GRAB THE CHISEL. THE OTHER EMPLOYEE STRUCK HIM WITH THE HAMMER. FINGER IS BROKEN AND REQUIRED SURGERY

Ee was stacking stone when he felt a pull in his back. The pain went up to his shoulder.

Employee was stacking stone on a pallet when a rock fell off and struck his right knee.

Employee was pulling a block of stone across a table to be sawn. He felt a pull in his lower back. Prescribed vicodin and naprosyn and placed on restricted duty.

Employee was stacking stone when part of it broke off and landed on top of the employee's foot above the steel toe. diagnoses with a contusion and given naproxen and vicodin.

EE COMPLAINED OF A SLOW ONSET OF BACK PAIN. SENT HOME TO REST WITH INSTRUCTIONS TO SEE DOCTOR IF PAIN CONTINUED OVER THE HOLIDAY. EE WENT TO DOCTOR ON SATURDAY FOR X-RAY. PRESCRIBED VICADIN

EMPLOYEE WAS STACKING STONE WHEN HE SMASHED HIS MIDDLE FINGER.

EMPLOYEE WAS STACKING STONE WHEN HIS BACK "LOCKED UP."

EE WAS ATTEMPTING TO CUT SOME STONE ON A STONECUTTER BY MOVING IT INTO POSITION WITH A PRY BAR THE ROCK SLIPPED AWAY FROM THE EE CAUSING HIMT O TWIST FORWARD.

STONE FELL OFF PALLET & SMASHED HAND, RIGHT HAND.

ACCIDENT HAPPENED IN THE MAINTENANCE SHOP UNDER CONTROLLED CONDITIONS. INJURED LEFT WRIST WHILE TRYING TO REMOVE A BEARING USING A HAMMER AND PUNCH.

EMPLOYEE WAS LIFTING A BUCKET OF CLEANING WATER. SHE FELT A STRAIN/PULL IN HER RIGHT ARM.

EMPLOYEE WAS WALKING AROUND A CORNER. A CO-WORKER WAS TOSSING A PIECE OF STONE INTO A BIN. THE STONE HIT THE EMPLOYEE ABOVE THE RIGHT EYE. TREATMENT CONSISTED OF STITCHES AND ANTIBIOTIC OINTMENT.

TOOK WHEEL OFF TRUCK. LEANED WHEEL AGAINST BLOCK WALL. WHEEL TIPPED BACK OVER ONTO HIS LEG.

EMPLOYEE CAUGHT HIS FINGER ON A WOODEN PALLET. TREATMENT CONSISTED OF STITCHES.

EMPLOYEE WAS GETTING OUT OF A TRUCK. HE STEPPED ON A BOARD AND THE BOARD ROLLED CAUSING HIM TO TWIST HIS ANKLE. TREATMENT CONSISTED OF NAPROSYN.

EMPLOYEE GOT HIS HAND CAUGHT BETWEEN A WEDGE AND A ROCK.

EMPLOYEE WAS ATTEMPTING TO PICK UP A ROCK. IT SLIPPED AND FELL ON HIS RIGHT THUMB.

EE WAS BEARING STONE WHEN HIS FINGERS WERE CAUGHT IN BETWEEN THE STONE "STOPPERS" & ITS OPENING ON THE OFF BEARING SIDE OF THE TABLE.

EE WAS ATTEMPTING TO PRY DOWN A TOOTH THAT WAS STUCK ON STONE CUTTER. WHEN 3' IRON BAR HE WAS USING SUDDENLY CAME LOOSE, SMASHING HIS FINGER BETWEEN THE BAR AND THE METAL OFF BEARING LEDGE.

NOT CLEAR, LOWER BACK STRAIN.

HE WAS DISMOUNTING A FRONT-END LOADER AND STEPPED ON A ROUND-SHAPED ROCK WHICH CAUSED HIS KNEE TO TWIST.

EE WAS CARRYING A BOX OF HEAVY TOOLS 80 LBS DOWN A STEEP, ROCKY SLOPE WHEN HIS FOOT SLIPPED CAUSING HIM TO FALL AND THE TOOLBOX FELL ON HIS HAND.

WHILE EE WAS SPLITTING SLABS WITH ANOTHER EE, EITHER THE WEDGE OR SLEDGE HAMMER THAT HE WAS UYSING HAD A SMALL PIECE FLY OFF AFTER STRIKING THE WEDGE WITH THE SLEDGE. THE PIECE PUNCTURE THE UP ER INSIDE OF HIS LEFT THIGH. A FOREIGN OBJECT WAS NOT REMOVED OR FOUND.

EMPLOYEE INSERTED FINGER UNDER A 12" X 12" STONE IN ORDER TO MOVE THE STONE INTO POSITION ALONG THE ROLLERS OF A STONE CUTTER. WHEN HE MOVED THE CONTROL STICK TO LOWER THE HYDRAULIC CUTTER ONT O THE STONE TO SHEAR IT, THE ROLLERS LIFTED CAUSING HIS RIGHT MIDDLE FINGER TO BE SMASHED BETWEEN THE ROLLERS AND STONE.

EE WAS ATTEMPTING TO WALK THE STONE OVER TO A PALLET WHEN HE FELT A PULL IN THE RIGHT SIDE OF HIS BACK.

WHILE EE WAS SPLITTING STONE, A PIECE FROM THE WEDGE EMBEDDED ITSELF INTO HIS RIGHT THIGH.

EE WAS DISMOUNTING FROM A TRUCK WHEN HIS FOOT SLIPPED FROM THE STEP. EE GRABBED ONTO BAR ON SIDEOF TRUCK AND FELT A PULL IN HIS ARM.

STACKING SANDSTONE DRYWALL WHEN THE STONE SLIPED AND CAUGHT HIS MIDDLE RIGHT FINGER UNDER THE STONE.

WHILE OFF BEARING STONE FROM HYDRAULIC STONE CUTTER, EE FAILED TO REMOVED FINGERS FROM UNDER CUTTING TEETH BEFORE SECOND EE LOWERED THE CUTTER TO CUT ANOTHER STONE. FINGER WAS SMASHED UNDER TO OTH OF CUTTER.

EE WAS REMOVING STONE FROM CUTTER WHEN HIS HAND WAS SMASHED BETWEEN THE BAR & THE STOEN

EE WAS CHISELING STONE AND HIS HAMMER MISSED THECHISEL, EE INSTEAD STRUCK THE INSIDE OF HIS LEFTKNEE.

EE WAS MOVING (WALKING) A LARGE STONE TO PLACE IT ON A NEARBY PALLET. WHILE HE WAS MOVING IT HE TWISTED HIS BACK AWKWARDLY AND FELT A TWEAK IN HIS BACK. AFTER PLACING THE STONE ONTO THE PALLET HE FELT A PULLING SENSATION IN HIS BACK. SHORTLY AFTERWARDS, THE EE DESCRIBES THAT HE WAS HAVING "MUSCLE SPASMS" AT THE MIDDLE TO RIGHT SIDE OF HIS MID-BACK.
