Quarry worker swung the hammer trying to split a rock, and missed the wedge, hit their left pinky finger.

Quarry worker, was cutting rock with the grinder, wiped EE's safety glasses with EE's glove and took them off, and wiped EE's eye while getting the silt off EE's face with EE's glove, which EE thought was clean and pushed debris into EE's eye.

Employee was operating the Saw shop Corner machine. While readjusting the rock, the table slid causing a right-hand laceration to the top of the right hand getting caught between the rock and the saw.

On loading a stone onto the stone cutter rollers, stone slab caught the corner of the table and tilted causing reaction to tilt the rock back onto the rollers, and in the midst of trying to maneuver the rock back onto the rollers, the slab caught the worker's middle finger resulting in an injury.

EE was unloading rock with the forklift. EE tried to push the rock from the forks by hand and caused their finger to get caught which caused a laceration to EE's left index finger.

On 03/24/2021 employee let me know that they had been having some pain in right wrist. Employee said that the pain started a week prior and they got a wrist brace for it, and the pain still continued to get worse. Employee splits stone and uses a sledge hammer with a chisel to do so.

EE was chiseling stone and got a piece of metal from EE's chisel in EE's leg.

EE was using the grinder when EE felt the grinder started to feel like it was becoming loose. The kill switch was located directly near the grinding wheel and guard. EE was slowly moving the grinder down towards the ground so the wheel wouldn't hit EE. As EE did the wheel came off and so did the guard.

Employee was moving some rocks around and felt a strain in the back. As the week progressed, EE reported that the pain started to get worse and EE could feel it in the leg.

EE was hammering a part trying to get a bearing off of a pulley, when a little piece of metal came off of the punch and hit ee in the left side of the chest.

EE reported to us on 11/21/2017 that EE had been feeling some abdominal pain. EE had a hernia two years ago which required surgery. I made an appointment for the EE right after Thanksgiving break.

EE was grinding on a bolt cutting, the cutting wheel disintegrated and a small piece of metal got in EE's eye. EE was wearing safety glasses at the time of the accident.

Fall from machine

EE was walking back to loader which was parked near a 5ft ledge. As EE was climbing in EE placed hand on the loader to get in, slipped and lost balance. EE fell backwards down the ledge.

Employee was walking down an area where rock had been quarried and ground was unstable when they slipped down and fell back on their hands. They injured their right hand due to trying to catch their fall.

Employee was moving slabs from on spot to another when employee got employee's finger caught in between two and split it open.

There was a low drawer open in the office and employee ran into it injuring employee's shin. Employee had some major swelling and bruises.

Employee was operating a Cat 963 Track Loader and backed into a 4 ft. hole. The loader rolled over in the hole, there was no injury or damage to the machine.

EE was in the quarry walking to fix a cutter and slipped on some ice. When EE fell, EE landed on hands to try and catch self and ended up injuring left shoulder/pectoral.

EE has been an employee here with the company for about 6 years. EE started feeling a pain in lower stomach area and reported it to supervisor, which supervisor gave ee a back support for to help. EE reported a week later that the pain was getting worse.

EE reported on 10/03/2016 that they felt some pain in abdominal area. EE said they felt a bump in May but it did not bother them until now. EE had a hernia the previous year and thinks they may have another one.

Employee was off bearing and pulled a rock closer towards him, didn't realize the rock was heavier then expected and it fell on his foot. He was wearing steel toed boot but he feels some pain in his foot.

Employee had came into my office on 8/27/2015 explaining to me he had injured his finger the week before and the swelling had not went away. He was lifting up stones and placing them down, he had got his finger caught under one as he was placing it down causing an injury to his right pinky and ring finger.

Miner was rolling a rock towards a scrap pile when he picked it up to place in the pile he felt a pain in his right abdominal area. Pain was a 3 on a scale where 1 is the lowest. He finished his work shift thinking that rest that evening would take care of problem, but still had pain when he returned to work the next day.

Worker was splitting rock with a hammer and chisel when he heard a popping noise and felt pain in his wrist. He finished his shift and then performed other duties the next 2 work days. When he attempted to split rock on the 3rd work day he had pain in his wrist.

Employee was placing stone onto conveyor system for thin cut saw when the piece split into two pieces and one of pieces fell on top of his right foot between the steel toecap and the ankle. Employee was sent for X-rays, which were negative and told to return to work on Wednesday.

Worker was holding a rock on cutter #46, when the stone was snapped his foot slipped and he felt a pain in his ribcage. His supervisor saw him asked him if he was hurt he said that he was, but he thought it would get better over the weekend so he didn't wanted to go to the doctor, but on Monday he told his supervisor that his ribs still hurt and he wanted to go and see the doctor.

Employee was off bearing stone from a stone cutter. There are two forks that come up from the table to ""catch"" the stone. The stone jumped and the employee caught his little finger between the stone and one of the forks. He received 7 stitches and was prescribed hydrocodone.

The ee was climbing out of the excavator and slipped on snow from the tracks and landed with his left hand on the rail on the left side of the track. He was prescribed vicodin.

The doctor stated that there was not a pinpointable accident, but a cyst possibly caused by overuse. No restrictions were imposed but the employee was referred to an orthopedic specialist.
