Fall onto or against objects

EE was moving a large cap stone, by standing it upright and walking it to the pallet. EE tripped or stumbled and fell. As EE fell the stone smashed EE's thumb between the stone EE was moving and one that was on the pallet.

EE was stacking a pallet of flagstone onto a pallet and a piece that he was handling broke in two. As a result, ee lost his grip and the broken piece came down and smashed his right middle finger. EE received a laceration and crush injury to his finger requiring sutures and a finger brace.Carmelo was wearing all of the required safety equipment including gloves.

EE was standing next to an exterior metal storage cabinet when a very strong gust of wind blew the door open. The edge of the door struck ee on the right anterior portion of his cranium, resulting in a laceration.

Fall onto or against objects

As EE was carrying stones over to a pallet that was on level ground, he tripped and fell forward. When he fell forward, he dropped the stone in front of him and used his hands to catch his fall. When EE extended his arms, his right forearm scraped the edge of the stone he was carrying. As the result, he sustained a laceration to the inside of his right forearm.

A rock caught his Left pinky finger between the rock and a steel bucket used for transporting rock while loading the bucket. He went to the hospital for an examination and then returned the next morning to work. The Dr. gave restrictions but he has not lost any days of work.

Struck by falling object

Large rock fell on left foot just below ankle. Was wearing steel toed boots but rock hit side of foot pinching between rock and steel track. Tried calling MSHA person for questions on 3/19/09 and received answers on 3/31/09 due to phone tag and serious illness in reporters family.

Struck by flying object

EE reported an object in his l. eye. He said that it happened on 12/29/08 when asked. He said that something had blown into it & he had been trying to wash it out for a couple days. He also said he was wearing safety glasses at the time. He went to the hosp. to have it looked at & the Dr. didn't find anything wrong at that time.

Employee was moving rock on cutting table, when rock started to fall from table. He tried to catch the rock to stop it from falling off the table, when he smashed the tip of his right hand index finger.

This employee was taken to the emergency room on 07/14/04 with complaints of pain in his leg and foot. After X-rays and scans of his lower region the doctor found that he has ruptured discs in his back. He could not pinpoint an exact date as to when the injury was incurred nor could he attribute the injury specifically to any one work-related task.
