AT 12:50 PM THE CORE DRILLERS LIGHTS WENT OUT. THE HELPER WENT TO CHECK THE CONTROL AND SAW SMOKE. THE DRILLERS WENT TO THE STATION CALLED THE HOISTMAN AND OTHER CREW. A SMALL 480 V TO 120/240 VOLT STEP-DOWN TRANSFORMER HAD FAILED. FUSE PROTECTION SHUT-DOWN POWER NO EVIDENCE OF FIRE. CREWS WERE EVACUATED.

AT INTERSECTION OF 528N AND 60' LONG 5X7 DRIFT 773 #1 RAISE, EE SAW THAT SLUSHER CABLE WERE CROSSED. SLUSHER WAS NOT RUNNIGN WHEN TONY ATTEMPTED TO TURN THE SHEAVE BLOCK 1/2 TURN. THE SLUSHER CABLES WERE TAUT. WHEN HE TURNED THE SHEAVE THE CABLE RAKED ACROSS HIS FINGERTIPS.
