2 mine operators- moving concrete blocks, 1 loader operator, 1 operator standing between concrete blocks hooking chains to 2 pick points on concrete blocks. (Blind spot) loader operator lifted bucket at request of operator hooking chains to blocks. Block swung forward into operators legs (pinch point) between blocks.
