Caught in, under or between a moving and a stationary object

EE was assisting in the tubing of the hole. EE's left 4th finger was caught between the drilling tube and the carbide edge of the drill bit when the tube suddenly bouyed up due to the water table. EE suffered lacerations to the finger.

EE (truck driver) was sitting in truck while being loaded with overburden and the load hit the truck bed and shook it violently causing lower back injury.

Going up wet stairs foot slipped. Fell down and twisted back. Stairs have clay and wet came off of peoples shoes.

Struck by falling object

EE was helping Mechanic remove the pulley that is attached to the shaft of the vibrator on the Rock Crusher. The bolts had been removed from the Pulley, for removal from shaft. EE was inside Hopper Frame removing belts from top & bottom Pulley's when top Pulley fell onto his left wrist causing a Laceration

Employee was using a wrench to tighten a bolt on Branford Mine scale. Wrench slipped, causing employee to almost fall down. Employee injured left knee while preventing fall.

Employee was lifting engine hood on excavator. The hood hinges were not working properly causing employee to jerk up on hood to open, which caused injury to lower back.

Fall to the walkway or working surface

Shop Mechanic was working on a Rock Crusher. When he tried to start Rock Crusher, the starter locked up and started to smoke. Shop Mechanic ran to his vehicle to get a wrench to unhook battery and tripped and fell on left shoulder causing chest pain on left side.

EE turned wrong while trying to sit in a chair at the desk in the mine office. Lower back strain.

Employee was entering bathroom in Mine Office. Door sticks and has to be lifted slightly by handle to open. Employee felt sharp pain in middle back when door was lifted to open. Pain is not in spine, but on each side of spine. Appears to be a muscle strain.

TRUCK DRIVER GOT OUT OF HIS TRUCK, UNKNOWN TO MINE SUPERVISOR AND STEPPED UP ONTO LOADER. IN THE PROCESS OF CLIMBING ONTO LOADER, THE DRIVER FELL AND CAUGHT THE CORNER OF THE STEP WITH THE INS IDE OF R LEG JUST BELOW THE KNEE CAUSING A PUNCTURE WOUND THAT REQUIRED 2 STITCHES. TRUCK DRIVER HAD PRIOR SITE SPECIFICS.

EE WAS SHOVELING ROCK FROM UNDER ROCK CRUSHER AND PULLED A GROIN MUSCLE. EE DID NOT SEEK MEDICAL ATTENTION UNTIL 7-23-01. EE WAS DX WITH A RIGHT INHGUINAL HERNIA SECOND DEGREE SHOVLING. EE WAS OUT FORE 42 DAY AFTER A 2"" HERNIA REPAIR.
