Denali Materials, Inc · Fall from machine

EE said slipped off the track when dismounting dragline. Later in the day EE went to Dr. Dr said no damage but has pre-existing conditions. Sent back to work. Later EE's back hurt. Returned to Dr & did physical therapy but no restricted duty or lost days. Received an MRI on 5/16/17. During MSHA inspection, Inspector & MSHA Supervisor said need accident report to start on 5/16/17.
