Tumbler shaft slid out of 4" nylon strap. Strap was not greasy and was in choke position
Installed inferior bearing into pillow block, bearing broke, piece went into left forearm.
Removing bolts from swing drum. Hit pinch with hammer and a piece came off punch, penetrating glove, and went into left hand.
Mechanic was working on a Volvo A30G Haul Truck with the bed in the raised position without it being blocked or secured to keep bed from accidental lowering. The mechanic was working on hydraulic system when EE loosened a hydraulic line letting the pressure off causing the bed to fall.
