Employee said he walked of ledge and fell 10 to 12 feet.
Was moving broken slabs with fork lift and couldn't get fork under piece. Got off fork and shoved slab on fork.
He was swinging a hammer on bottom line and turned the wrong way.
Was changing forks to bucket ad right side tilt arm wouldn't move so we raised bucket upa and was tilting at same time when right side tilt arm pin exploded, and hit him in the arm and waist.
WAS HITTING WOODEN WEDGE WITH HAMMER, TO LEVEL TRACK, MISSED WEDGE & HIT THUMB.
