Struck by falling object

Miner was processing a slab of stone on the hydraulic splitter when EE rolled the stone back a piece fell off and hit EE's steel toed boot on top and bruised the top of EE's foot.

Struck against a moving object

Employee had a piece of stone on the forks of a skid steer getting ready to load the stone splitter table when EE lost control of the machine and stone came off making skid steer rock back and forth severely and allowing employee to hit the lap bar / hold down bar with EE's chest fracturing a rib

It was a dry warm day and Employee was running a hydraulic stone splitter making Dimension type stone for Landscape type rock (Flatrock). When splitter came down to split stone a small piece hit Employee in right cheek causing Employee to fall backward and hit the back of Employee's head on a pallet causing cut to the back of head as well.

While holding a piece of stone on our Hydraulic stone splitter to be snapped / split into dimension stone. When stone was snapped it jarred EE's Right hand / wrist. Thinking EE's wrist was only sprained EE continued to work the rest of the day. Later that evening while at home EE's wrist continued to hurt and EE went to the hospital and Dr's determined it was broken.

Struck by falling object

While feed table was being loaded with flat rock the skid steer operator pushed the table of stone / flat rock forward and a piece of stone fell off causing it to hit victim on the left foot / ankle. Employees are taught to stay away from table while being loaded.

Employee / Crew member decided to load a piece of stone on a 3ft splitter table by hand instead of waiting on a machine (skid steer) to load table, resulting in a pulled muscle in his lower back.

Over-exertion in pulling or pushing objects

While pushing flat rock / landscape stone to the 3 ft splitter to dimensionalize stone, employee felt pain & discomfort in left shoulder. He worked till lunch at 11:00 am and then decided to go to Dr. due to discomfort / pain in that shoulder. Conditions in that area were dry, partly cloudy and 30 degrees.
