Climbing up ladder into haul truck, foot slipped pulled shoulder of arm holding on with. Employee continued to work did not require medical treatment until 09-08-15 and has been off since then. Expected to return on November 9th 2015.
Drill operator was helping blaster load shot. Blast hole was being loaded with 13 pound sticks. While reaching for a stick the driller states his back "went out". Originally diagnosed as a sprain & no lost time at work. Condition worsened after physical therapy. Re-diagnosis revealed bulged disc. EE is restricted to drill operating only & has no physical labor.
MINER WAS CLEANING OUT A HOPPER WHEN A LUMP OF MATERIAL HE WAS DIGGING CAME LOOSE AND ROLLED IN TO HIS LEG STRAINING HIS KNEE.
DRILLER WAS UNSCREWING DRILL ROD WITH A WRENCH AND MASHED HIS HAND BETWEEN THE WRENCH AND THE DRILL MAST.
