EMPLOYEE WAS OPERATING A CUTTING MACHINE IN THE NUMBER 5 HEADING. AS HE BEGAN HIS SECOND CUT, THE ROOF STARTED COMING IN. HE WENT TOT HE FRONT OF THE CUTTING MACHINE WHERE HE WAS TRAPPED BY THE FALL. THE BOSS, HEARD THE FALL AND SUMMONED THE STATE AND FEDERAL AUTHORITIES.

VICTIM WAS OPERATING A SCOOP AND AS THE SCOOP ARTICULATED, THE VICTIM LEFT HAND WAS PLACED INADVERTQNTLY IN THE PINCH POINT AREA AND THE 2 AND 3 FINGERS OF THE LEFT HAND WERE PINCHED CAUSING A MPUTATION AT THE LAST JOINT DISTAL TO THE HAND INFORMATION FROM VICTIM ABOUT INJURY.
