OPERATION OF MINER WAS TRAM MIENR. HE WAS IN PROCESS OF BACKING UP. TRAN MALFUNCTION AND MINER DID NOT STOP AND ONCE ON THE OFF SIDE. MINER THEN PUITED AND CAUGHT OEPRATIONS FOOT BETWEEN RIB AND MINER.

HE HAD CUT THE FACE AND WAS BACKING MINER TO THE OUTSIDE. HE WAS WALKING BESIDE OF MINER THE MINER SLEWED TO THE SIDE AND CAUGHT HIS LOWER FOOT OR ANKLE AND BROKE IT.
