Struck by falling object

The miner & a co-worker installed blocking material under the cutter head of the continuous miner to support it while they replaced the load-lock valve. The victim was in the process of installing the new valve when the blocking material failed & allowed the cutter head to fall to the mine floor. The victim was crushed between the boom of the cutter head & the frame of the pan.

Employee was lifting a belt roller to put on a pallet and felt a sharp pain in his lower back.

Employee was opening a set of Kennedy doors to allow a trip to go through. He closed one side of the doors and while he had his back to the other door it swung closed and caused him to hit his head on the first door resulting in him having to have stitches in his fore head.

Employee was bolting on #1 unit in #2 left swinging over to put up his 2nd pin in his 2nd row when a rock slid off of the canopy and hit him in his lip and arm causing him to receive 4 stitches in his upper lip.

A roof fall occurred on the 3rd 54 belt line 3rd main east xc 66 #5 entry. The fall measured 110'x 19.5 x unknown and was caused by a hidden slip in the mine roof. The area was mined in May of 2008. There were no injuries, no equipment involved and ventilation was not affected.

Struck by falling object

Employee was helping change bits on the miner when a small piece of rock 2 1/2"" X 2 1/2"" in size fell out and struck him on his left forearm causing him to have 3 stitches.

Employee was reaching across the belt to shut it off when he slipped and fell onto the belt and had to swing his self off with his right arm causing a tear in his bicep. He continued to work till his surgery on 8/17/15.

A roof fall occurred in the 2nd 54 intake 3rd main east x-cut # 38 in the # 7 entry. The fall measured 20' x 20' x 6' and was caused by a hidden slip in the mine roof. The area was supported by 6' tension rebar bolts. The area was mined in April of 2008. There were no injuries, no equipment involved and ventilation was not affected.

Employee was taking belt chocks back to load on the ride when he felt a pain in his lower back, he continued to work until the doctor put him off work on 8/26/15.

Employee was removing a screen panel from the refuse box when he slipped and mashed his right index finger resulting in a hair line fracture of the end of the finger.

Employee was walking to the roof bolter in # 2 entry when he ran into a roof bolt that cut his left shoulder that caused him to have 6 stitches.

Employee was bolting in the face of # 5 entry on # 3 unit when his steel stopped up and he hit the steel with a 1 foot wrench to unstop it, the wrench bounced back and struck him over his right eye causing him to get 6 stitches in the cut.

Employee was making a pin up at the back of the bolter when he turned to go up to the front of the bolter he hit his back on a roof bolt causing him to cut his back. He finished his shift and went to the doctor the next day and received 6 stitches.

EE was scooping out by #2 unit when he noticed the rib he had just scooped was loose, so he jumped out of the way and the rib caught him on the back of his lower legs. He only had contusion to both legs but was not given a full duty release by the doctor.

Employee was turning the scoop he was driving into a crosscut when his hand slopped off of the side of the scoop and was caught in the pinch point where the scoop swivels. Resulting in the fracture of the end of his index and pinkie fingers on his left hand causing him to be off work.

The area was supported by 6' tension rebar roof bolts in the pillar line. The area was mined in March of 2008. There were no injuries, no equipment involved and ventilation was not affected.

Employee was trying to get the reel of the roof bolter on # 2 unit in # 8 entry unstuck. He had his hand on the reel when two employees pulled the bolter cable and the reel became unstuck which pulled his left hand into the reel causing him to have stitches.

The area was supported by 5' fully grouted bolts and 10' cable bolts in the intersection. The area was mined in April of 2014. There were no injuries, no equipment involved and ventilation was not affected.

Employee was cutting some metal outside at the shop when he tripped and fell on his left elbow. He continued to work until he was off work with surgery on his elbow 08/08/2014.

Employee was cutting # 7 left on # 1 unit when a rock fell out grazing down his left shoulder causing him to yank his arm and shoulder away from the rock. he worked until he saw a doctor and had X-rays which showed nothing wrong. The Doctor keep him off work starting on 5/15/14 for therapy.

Employee was throwing the miner cable over on #3 unit in #6 right when he felt a sharp pain between his shoulder blades. He continued to work till he was put off work for therapy on 5-19-14.

Employee was operating a mini trac on #2 B supply road when he ran over a rock in the road which caused him to jam his head and neck into the roof. He kept working till the doctor put him off work on 3/11/2014.

Employee was standing at the #1 unit feeler in #4 entry with his back to #5 entry. A ram car came to the feeder from #5 entry side to dump. The employee at the feeder did not see the car coming nor did the ram car operator see the employee. The ram car pushed the employee down which resulted in a fracture to both his legs below the knee.

EE was stepping off of the mantrip at #2 unit shack when he tripped over his jacket and hit his head on a roof bolt causing him to have 4 stitches.

Employee was walking across from #7 to #8 entry on #3 unit when he felt his lower back pop. He had been loading pins and boards on the right bolter earlier that morning. This accident happened on 12/3/2013 he continued to work till he had back surgery on 07/15/2014.

Employee was sitting on a pallet of blocks with his back turned to a co-worker who was cutting plastic bands off of the block pallet, he turned his head to see if his co-worker was done cutting the bands and the cut band flew up and hit him in his right eye causing a cut in his cornea.

On July 24, mine experienced an adverse geological event, including fall of roof in alive workings, that impaired ventilation or impeded passage within the 6th Sub Panel East off 1st South Panel. No injuries were involved and MAHA was contacted within 15 minutes.

He had stepped back to the middle of the bolter to get a bundle of plates when the operator yelled to him that he was moving up and the bolter tire ran over his right foot causing him to break his toe.

Employee was driving a spad in the rib to hang wire for cables on #2 unit in #5 entry when he hit the top causing a piece of top to fall out and hit him in the side of his left foot. This resulted in a hire line fracture of his foot.

Employee was helping put pins and bushings in the M-7 miner on #1 unit in #9 entry when a co-worker was driving the right bushing in a piece of the bushing broke off and struck the employee in the neck. He was taken to the doctor and the metal was taken out, with one stitch put in the incision.

Employee was plastering a brattice between #4-#5 entry across from the feeder. He had his stinger ride parked between him and the feeder with the lights on. A ram car backed into the stinger and pushed it up on top of his legs. Resulting in a fracture to his pelvis and his lower back.

Employee was pulling cable off of the reel of the roof bolter when he twisted and felt a sharp pain in his lower back. He was between #7 and #8 entry in the last open cross cut on #1 unit. He came back to work the next day.

A roof fall occurred in the 3rd Main East intake between x-cut 30-31 in the pillar line #7 intake entry. The fall meas. 25'x6'x19' and was caused by a hidden slip in the mine roof. The area was supported by 6' tension glue bolts and was mined in 3/08. There were no injuries and no EQ. involved and ventilation was not affected.

Employee was lowering the lid on a set of ram car batteries when the lid fell on his left index finger which caused a fracture in the tip of the finger and 6 stitches.

Employee was getting down from a end loader when he stepped on the ground his foot came down on a tire rut, causing him to turn his ankle.

EE stated he was grading the hall road and was stopped waiting for the artic. trucks to move. He finished his drink and opened the door and threw the can out, as he was shutting the door he shut it on his left hand. The door striker that latches caught his left middle finger on the inside had a laceration. EE had 15 stitches in E.R. Minor injury.

Employee was cutting a shock off a mantrip in the shop when the shock ruptured and burned him on his right arm and face. He was treated for second degree burns on his arm.

Employee was holding a wing up that had been torn off of a feeder when he lost his grip on the heavy metal wing causing it to fall and smash his tight little finger, he had 8 stitches in his finger.

He was traveling batteries first and could not see the employee that was hanging the curtain. The rear tire of the scoop ran over the right foot of the curtain hanger causing a fracture of the small bone in the ankle.
