Cleaning up and lifting-moving conveyor rollers strain sprain low back
Back strain from installing roller on conveyor.
Slip trip fall - employee was stepping backwards on ground removing a roller rod from the screen when EE tripped and fell cutting finger.
MAINTENANCE WORKER WAS USING A "COME ALONG" RATCHET STRAP TO PULL A TAIL PULLY AND THE STRAP SHIFTED CAUSING THE EMPLOYEES END OF FINGER AT NAIL TO BE MASHED
Miner was on foot and walking in the travelled path of a caterpillar 980G loader during a period of heavy rainfall and overcast conditions and was struck by the loader bucket, resulting in a laceration to the scalp and a broken pelvis.
Removing torque on D400 Truck. Using 1/2 impact wrench to get bolts loose. Went to reposition to get to other bolts & lost grip on impact wrench. It fell hitting him on the right side of the head
