Roof Bolting and pulled lever and canopy did not lower. Employee bumped lever and canopy fell on employee.

Putting a foot shaft in miner and it fell against sprocket on pot and caught finger.

Repairing ram car wheel. Another employee operating separate ram car struck a 4 wheeler and then it rolled into the ram car EE was repairing striking him in leg.

Assisting the mechanic on the repair of the miner. Mechanic was prying on the motor and it pinched EE's finger causing laceration.

Bolter steel fell striking employee on 5th finger on left hand.
