Steelstone Industries I · Fall from machine

Employee was welding on the wash rack. After he unhooked his safety harness to reposition himself, he slipped on some gravel and fell 16 feet to the ground below. There were no witnesses to the incident, he called on his cell phone to report the fall to a co-worker. He bumped his left temple and had a tail bone contusion.

PLACED CONCRETE PAD UNDER STONE BIN. EMPLOYEE WAS TROWELING CONCRETE WHEN A PIECE OF I BEAM SLID DOWN CAT WALK OVER HEAD AND STRUCK EMPLOYEE ON RIGHT SIDE OF HEAD. TWO EMPLOYEES WERE REPAIRING A CONVEYOR.
