Emp. removing cutting edge from loader. Emp was turning a bolt w/ an impact gun while holding cutting edge up w/ EE's left hand. The bolt head began to turn quickly. Said bolt head was sharp due to being ground down. Emp Left middle finger came in contact w/ bolt head, lacerating the finger through leather gloves.

Employee was going to put air in one of truck tires. EE pulled the air hose off of the reel and the other part of the hose was not connected. The male end of the hose came off of the reel, striking the employee in the upper lip. As a result two teeth were pushed back and subsequently fell out.
