Employee cut the tip of his finger off when trying to load a pipe into the back of a pick up truck.
Employee was struck in mouth with a pad eye. Employee received six stitches to his upper lip.
Employee felt discomfort to the right shoulder and bicep area when lifting & disposing of a wear cap weighing 79 lbs.
Employee received a laceration to his left forearm while drilling on a piece of stainless steel.

After hooking up the cable to the channel the employee fell and struck his head.
Employee chipped right front tooth. On 10/10/2012 we were made aware the employee had the tooth filled with resin.
Employee was carrying a box and the box began to slip. As the employee grabbed the box, he felt a pull in his upper right arm. Restrictions began 08/02/2012.
Employee was a degenerative disc condition as well as bad knees with three knee replacements. These conditions make him prone to back and knee injuries. Employee was moving a box of rain jackets and felt pain in his lower back. He stated the pain was similar to the incident on 02/27/2012. This is being submitted under protest.

Employee got his finger caught in a pinch point while putting a chain on a sprocket.
Employee slipped on a mine road on 4-16-11 and strained his shoulder. Although his condition improved, he did not completely heal. Physical therapy began on 5-25-11. Safety was notified on 5-31-11.
While employee was removing a hydraulic oil tank drain plug, he felt soreness in his neck.
Employee pinched his finger between his wrench and a drill mast motor when his wrench slipped.
Employee slipped on ice after exiting his vehicle and hit his head against the ice.

While an employee was removing the shovel hoist drum locking lever, the lever pulled out from the drum faster than expected. He tried pulling back on the lever and it stretched out his left arm, causing pain in his left bicep and elbow.
Employee reported to work the day after he removed a pump at the utility plant.
Employee was opertating a hand held grinder when the grinder kicked back, hitting him in the chin with its handle. This caused his mouth to close quickly, chipping an upper front tooth. Employee received a crown on 7/21/10.

A natural gas odor was present in warehouse at the start of the shift. The doors were opened and the odor cleared. Late in the shift, an employee felt ill and went home due to the odorant. Her work location was changed due to the exasperation of a pre-existing condition.
Employee received small sliver in his leg while attempting to remove a lip shroud using both a hardened steel hammer and a brass hammer.
While grinding slag from the edges of a floor liner plate, the grinder came in contact with the employee's leg.

Employee was walking towards his truck and slipped on the snow covered ground. Initially this was a no treatment case, but has been changed to a Restricted Work Case as of 1/12/2008. **Due to surgery, this case is changed to a Days Away case as of 3/6/2008.**
While employee was removing bolts from a grinder, dust blew in his eye causing irritation.
While exiting a dipper, EE slipped on shank, resulting in EE falling on his right hip and shoulder. Due to an unresolved back pain, the doctor recommended physical therapy as of 12/17/07.
While installing torque tool onto wheel nut, the torque tool slipped off of the nut causing the left 3rd finger to be pinched between the reaction arm of the tool and the rim of the tire. Employee changed jobs & has restrictions in his new position. However, the restrictions were short lived & did not affect his job.

ON 3-20-03 THE INJURED WAS CLOSING A HYDRANT, HEHAD LOOSEN A 2 1/2"" CAP TO RELIEVE THE PRESSURE THE CAP CAME OFF & STRUCK HIM & THE FLOW OF WATER THREW HIM BACK AGAINST A PARKED TRUCK. ON 3-26-03 HE BEGAN LOOSING TIME.
