After hooking up the cable to the channel the employee fell and struck his head.

Employee chipped right front tooth. On 10/10/2012 we were made aware the employee had the tooth filled with resin.

Employee was carrying a box and the box began to slip. As the employee grabbed the box, he felt a pull in his upper right arm. Restrictions began 08/02/2012.

Employee was a degenerative disc condition as well as bad knees with three knee replacements. These conditions make him prone to back and knee injuries. Employee was moving a box of rain jackets and felt pain in his lower back. He stated the pain was similar to the incident on 02/27/2012. This is being submitted under protest.

Employee slipped on a mine road on 4-16-11 and strained his shoulder. Although his condition improved, he did not completely heal. Physical therapy began on 5-25-11. Safety was notified on 5-31-11.

While an employee was removing the shovel hoist drum locking lever, the lever pulled out from the drum faster than expected. He tried pulling back on the lever and it stretched out his left arm, causing pain in his left bicep and elbow.

Employee was opertating a hand held grinder when the grinder kicked back, hitting him in the chin with its handle. This caused his mouth to close quickly, chipping an upper front tooth. Employee received a crown on 7/21/10.

A natural gas odor was present in warehouse at the start of the shift. The doors were opened and the odor cleared. Late in the shift, an employee felt ill and went home due to the odorant. Her work location was changed due to the exasperation of a pre-existing condition.

Employee was walking towards his truck and slipped on the snow covered ground. Initially this was a no treatment case, but has been changed to a Restricted Work Case as of 1/12/2008. **Due to surgery, this case is changed to a Days Away case as of 3/6/2008.**

Fall from machine

While exiting a dipper, EE slipped on shank, resulting in EE falling on his right hip and shoulder. Due to an unresolved back pain, the doctor recommended physical therapy as of 12/17/07.

While installing torque tool onto wheel nut, the torque tool slipped off of the nut causing the left 3rd finger to be pinched between the reaction arm of the tool and the rim of the tire. Employee changed jobs & has restrictions in his new position. However, the restrictions were short lived & did not affect his job.
