#79 Pioneer 50 VEMetal/Nonmetal

Employee was walking electrical cord backwards and tripped over bobcat bucket that was parked near the working area. Employee fell backwards and landed on their backside. Employee has a very sore buttocks and a headache set in right away. Employee is continuing to work at this time.

Clearing a jam in the crusher and the pry bar slipped and broke employee's left pinky finger

Walking out of crusher shack and stumbled and broke right ankle

Cleaning out rolls on crusher, standing on belting slipped metal edge hit right ankle rolled ankle cracked R. foot.
