Struck against stationary object

Labor was setting up blocks to be sawed. Stepped off the saw cart employee was standing on. Stepped on to a small rock on the floor and twisted right ankle.

Struck by flying object

Worker was chunking rock while a burner was being used. Worker was wearing all PPE. However a hot rock got into Left eye. Called office and getting into eye doctor at 2:00 today.

Struck by falling object

Employee was helping another employee change the drill bit on a jackhammer when the other employee dropped the jackhammer on the employee's right hand, mashing his middle finger on his right hand.

EMPLOYEE WAS STEPPING UP ON A LEDGE WHEN HIS FOOT SLIPPED CAUSING HIM TO SCRAPE HIS RIGHT FOREARM ON THE EDGE OF THE LEDGE.

Employee was leveling a tamrock drill and got his thumb under the outrigger and mashed his thumb.

Employee was drilling on a rock and he stepped on a piece of grout rock that moved causing him to fall, bending his knee at an awkward angle.

Struck by falling object

Employee was moving granite slabs with an overhead crane when one of the slabs broke in the middle falling about 5ft. A small piece broke off the edge and hit the employee in the right cheekbone.

Over-exertion in lifting objects

EE was placing grout (scrap granite) in a pan to clean up his work area. He picked upa 50 pound piece and turned to place the granite in the pan when he felt a pull in his lower back area.

Fall onto or against objects

Ee was standing on a large block of granite, trying to break off a piece of the rock. When the piece of ther ock broke off it caused the block to shake slightly. He felt he was losing his balance. He dropped down to his knees to catch himself and struck his knee on the same granite rock. His fall was from a standing to a kneeling position.

Fall from machine

Ee is not an employee of missouri red. he went to flip a 4"" strp loose from the trailer. the strap did not unhook, causing him to lose his balance. he fell approx. 4 feet to the ground, laning on his right side.

Ee was pushing the compressor shed door (sliding door), door became jammed in the track - He gave a quick shove on the door. The door did not give and ee was twisted at the waist. He felt a sharp stab of pain in the middle of his back.

Struck by falling object

PULLING A BLOCK WITH THE LOADER & HOOKS. A HOOK BROKE OUT OF THE BLOCK BEING PULLED AND THREW A PIECE OF ROCK IN THE AIR. IT FELL HITTING EE IN THE BACK OF HIS HEAD. EE RAN AWAY WHEN THE HOOK BROKE LOOSE.

EE WAS DRILLING A HOLE WITH A JACKHAMMER ROCK DRILL WHEN IT BECAME STUCK. HE TRIED TO PULL THE DRILL FREE AND HAD TO ""PULL HARDER"". HE FELT A SHARP PAIN IN HIS BACK. HE ADMITTED TO LIFTING WITH HIS BACK AND NOT HIS LEGS. HE SUFFERED A PULLED MUSCLE IN HIS BIDDLE BACK.

EE WAS DRIVING WEDGES ON A CAP LINE, WORKING ON A PLATFORM ABOUT 5' OFF THE GROUND. WHILE HE WAS DRIVING THE WEDGES WITH A HAMMER THE CABLE SLIPPED OFF ONE SIDE CAUSING HIM TO FALL.

EE WAS LIFTING AN OXYGEN CYLINDER WITH A CO-WORKER INTO A FORD RANGER TO AMOVE TO ANOTHER AREA OF THE QUARRY. HE FELT A SHARP PAIN LOW IN HIS BACK.

EMPLOYEE WAS PRYING ON A GUARD WITH A PRYBAR. HE FELT A JERK IN HIS BACK DOWN LOW AND TO THE LEFT. THE PRY BAR WAS @ 4' AND THE GUARD WAS @ 250 LBS.

EE WAS MOVING A GRANITE SLAB TO HIS WORK STATION. WHILE LOWERING THE SLAB, HE GRABBED THE HOOK, PLACING HIS FINGERS BETWEEN THE HOOKS AND THE 5 LAB PRIOR TO THE RELEASE OF ALL THE TENSION CAUS ING HIS FINGER TO BE CAUGHT FINISH LOWERING THE SLAB AND REMOTE HIS HAND.
