Employee hit EE's left index finger with a sledge hammer while securing a metal screening unit part with EE's left hand and striking it with the hammer in EE's right hand. EE mashed the left index finger resulting in a fractured bone.
Employee was welding when a peice of metal flew into his right eye. Safety glasses and welding hood were both being worn.
