Miner was on first floor of ""New Office"" working on a ladder changing a light. Miners co-worker went to basement to retrieve another new light. Co-worker heard a thump and a bang on the ceiling. Co-worker found miner sitting slumped on the floor unresponsive. Started CPR, called 911, deployed AED but the miner could never be revived.

Miner was spinning off fuel filters when the tip of their left middle finger got pinched between the filter and the filter housing. This resulted in a non-displaced tuft fracture of left middle finger

Miner was dismounting an excavator to help fuel. Miner placed right foot on the ground first not seeing a stone covered with dust. Miner rolled right ankle which resulted in a fibular ankle fracture.

Miner reached around and below the truck seat to pick up an ink pen. Upon returning arm to a normal driving position, miner felt a sharp pain in right shoulder. This has resulted in a strain of the right shoulder.

Miner was swinging a 4 pound hammer attempting to dislodge an object when they felt a tightness in their right shoulder. This resulted in a strain to the right shoulder.

Miner was climbing a ladder/stairs to access the top of a portable fuel tank to re-fuel. Miner slipped on steps and fell. This resulted in a hyperextension, strain, and contusion to EE's left knee.

Miner was helping pull and maneuver a steel rope cable when a burr on the cable caught the miner's arm. This resulted in a laceration requiring sutures.

Miner had just started climbing up the ladder on the loader when the employee felt a ""pop"" in left wrist. This has resulted in a torn ligament.

Miner was climbing up into haul truck when foot slipped off a rung of the ladder causing miner to fall. This resulted in a fracture of left wrist.

Miner was fueling an excavator. Mud on miner's boots and track, caused miner to slip and strike left ribcage on the track. This resulted in a contusion and pain associated with a ribcage injury.

Miner climbed off haul truck to the ground. As EE was walking, EE tripped and fell. This resulted in a right wrist fracture.

Miner was working in a close-quartered area when EE turned around and impacted EE's left eyebrow on a beam. This resulted in a laceration requiring sutures.

Miner was dismounting haul truck at end of shift. EE's left foot landed on an uneven surface and EE rolled EE's left ankle. This resulted in an acute sprain/strain to the left ankle.

Miner fell from a ladder approx. 4 to 5 feet as EE was descending. Miner landed on a concrete floor. This resulted in muscle soreness and a concussion. Lost time began on 06-02-2023

Miner was on a loaded rock barge measuring freeboard when miner tripped on a rock and fell on the loaded rock pile. This resulted in a fracture of the right radius head near the elbow.

Miner was pulling an extra long length of hose off of a reel at somewhat of an angle. As miner's body was positioned sideways, miner felt a pop in right knee resulting in an injury to right MCL.

Miner was torching out a loader bucket liner when the liner unexpectedly popped loose from bucket. A corner of the liner struck the upper left shoulder causing a hematoma and puncture wound requiring sutures.

Miner was climbing a ladder to a conveyor crossover. A large rock rolled off the side of the conveyor hitting the rope switch cable which was directly behind the rung EE was standing on. When the cable snapped tight, it caught the toe section of EE's left boot which hyperextended EE's left knee.

Miner was climbing into haul truck when EE lost EE's footing. Miner fell to ground landing on feet. This resulted in a fracture to left ankle.

Miner was standing in an awkward, twisted position holding tension on a 5 foot pry bar. The bar jerked away from the miner causing them to be jerked in a twisted position. This resulted in an acute strain to the lower left back paraspinal muscle.

Contact with hot objects or substances

Miner was torching out a v-groove to repair a crack. A puddle of slag rolled off onto left forearm. This resulted in a 6X5 cm area of 2nd degree burn.

Miner was dismounting a Cat 375 excavator when they grabbed a door latch instead of stationary handrail. Door opened and high wind caught and swept miner off catwalk. This resulted in acute strains of rt. shoulder and knee.

Miner was climbing down ladder on Cat 730C when EE got EE's right foot lodged on a step and fell. This resulted in a contusion and hematoma to the left hip

Miner was descending the ladder of a haul truck parked at a barge loading facility. Miner slipped on a wet ladder rung causing miner to get foot twisted in the ladder. This resulted in an acute ankle/calf strain/sprain.

Miner was attempting to prime a 2"" gas powered water pump, pumping river water. When miner removed the plug, hot water spewed out on right hand. This resulted in 1st and 2nd degree scalding burns to the right hand.

Miner reported to maintenance ready line to grease equipment. While squat/walking under a haul truck, left foot slipped resulting in sharp pain in left groin area. This resulted in an acute strain of the left groin.

Struck by falling object

Miner was is the process of removing a bucket from a skid steer. When the final pin was loosened, the front of the bucket rotated down and landed on miners ankle. This resulted in a fracture of the lower rt distal fibula.

Miner was working under a piece of equipment and received a foreign body in EE's right eye. Ophthalmologist removed body with a spud and miner returned to work with a prescription.

Miner was using a chisel when a small piece of metal from the chisel end dislodged and became embedded in miner's left 3rd digit. Metal was removed at ER and the laceration was closed with 3 stitches.

Miner was walking on a decline when EE stepped on a slick spot on the ground. EE's left foot slipped and twisted as EE was falling. This resulted in a left distal fibula (ankle) fracture.

Miner was pulling a portion of a steel cable across a barge deck. EE suddenly felt a sharp pain in lower back. This was diagnosed as an acute lumbar strain. On 07-30-2018 it was determined ee was unable to work.

Miner was locating an electric motor in warehouse. EE slipped and struck right knee on the shaft of a motor sitting on the floor. After several weeks of monitoring and evaluating, it was determined on 03-13-2017 that surgery was needed. Surgery was performed on 03-15-2018.

Miner was loading 1' X 2' polyurethane panels when ee tripped and fell. EE attempted to catch fall with right arm and hand which was clenched in a fist. This resulted in a minor fracture in right pinky finger.

Struck by flying object

Miner was removing nuts from a metal belt splice clip using the manufacturer's designated tool when a flying sliver of metal embedded in their left forearm. After consultation on 09-21-2017, it was determined the sliver should be removed surgically.

Miner was using a utility knife to cut a section of 6"" hose from a flange fitting. Utility knife slipped resulting in a laceration to left ring finger. This required 8 sutures to close.

Struck against stationary object

Miner was exiting haul truck after parking in service/maintenance area at the end of shift. Miner swung door open and in an attempt to ""catch"" it, miner missed door, catching finger and bending it backward. This resulted in a fracture at the base of the right ring finger. Miner initially denied need for medical attention. First request for medical attention was 10-10-2016.

Miner had just finished fueling haul truck at Wash Bay fuel depot and was climbing back on the truck. As EE grabbed the horizontal grab bar below the window and started pulling self up further, EE's gloved slipped off causing EE to fall to the ground landing on EE's feet. This resulted in a fracture to EE's right heel bone.

Miner was helping pull a steel cable onto the deck of a barge. When everyone let go of the cable it started sliding back into the river knocking the feet out from under miner. This resulted in a strained knee, hip, and shoulder.

Miner electrician was walking around the back of a parked welding truck when he struck, with his forehead, the corner of a piece of expanded metal protruding out the back of the truck. This resulted in a laceration requiring stitches.

Struck against a moving object

Miner was traveling through plant area of quarry with a standard rubber-tired backhoe when he impacted an obstruction with the bucket. This brought the machine to a complete stop. This sudden stop resulted in face lacerations and a cracked rib.

Miner was helping to change wire cloth on shaker screen. Was prying up on screen panel when bar slipped and struck miner in forehead. This resulted in an 1 1/2"" laceration and contusion. Miner is also having headaches.

Miner was helping to install transmission cooler in Cat 777D. Was standing on a ladder helping to lift cooler in place which resulted in an acute twist and strain on his right knee.

Miner was found by co-worker laying unresponsive next to his excavator. Miner suffered a heart attack.

Miner was descending ladder on Cat 777 water truck being used for plant washout. Miner contacted the last rung ""on"" the heel portion of boot and didn't have rung locked ""in front"" of heel. Foot slipped through ladder causing miner to twist and fall striking left face, arm, and shoulder on ground. This resulted in several lacerations and abrasions. 1 requiring stitches.

Miner had just finished helping attach a steel winch cable to an empty barge being loaded at a belt loading dock. As he was walking empty handed to the stern of the barge, he grabbed his lower right abdomen and collapsed in pain. Miner re-injured an umbilical hernia that dates back to the year 2000.

Miner was helping to install a roll of new conveyor belting that had a steel shaft lifting rig through the center of the belt roll and supported by a crane. While pushing on belt roll to align with conveyor frame, miner smashed left ring finger between steel rig and a conveyor roller. This resulted in a tuft fracture to the tip of the left ring finger.

Miner was in vehicle maintenance wash bay cleaning out a truck bed using a 20' pike pole and a water hose. Miner jabbed pike pole into a small rock stuck in groove between welded plates. Rock didn't move jarring left hand and resulting in a sprain to the left wrist.

Lube mechanic was removing filter from engine during an oil change. Filter wrench slipped on filter and end of ratchet wrench struck miner above left eye. this resulted in a laceration requiring stitches.

Miner was helping clean snow and ice accumulation in and around a production plant. Slipped on ice and twisted right hip. This resulted in an exacerbation of sciatica. Miner continued to work and made first request for treatment on 12/14/2013.

Miner was attempting to remove a frozen nut from a mechanical belt splice using a chisel designed for the task. Upon striking the chisel, a sliver flew off and embedded in miner's left forearm. An incision and sutures were required for removal.

Miner was tighting nuts on wheel studs after wheels were removed for maintenance. Air impact wrench and socket slipped off nut when tightening an upper stud. While miner was trying to keep wrench from hitting ground, he strained his right shoulder and neck.

Miner welder reported pain and discharge from his right eye. This condition had been occurring for several days. Examination revealed a small piece of metal. Removed and prescribed an antibiotic. Miner is unsure of when and how metal entered his eye.

Miner was installing a planetary gear in a planetary housing on a 777C Cat haul truck. When gear fell into place it pinched the tip of his left middle finger. This caused a split finger nail and a small tuft fracture of the end of his finger.

Miner was loading muck pile with Cat 992G loader. Discovered a ""toe"" under the muck pile and was using loader to break out ""toe"". Drove loader into an area of ""toe"" that refused to move causing a very sudden and abrupt stop. This lurched the miner in his seat causing an acute neck and upper back strain.

Miner was in cab of Ford fuel truck assisting mechanic by ""bumping"" engine when necessary. Miner was wearing Tyvec coverall suit. Miner stepped out of truck and foot slipped on top step causing Tyvec pant leg to get caught on step. This flipped miner out of cab, landing on floor, resulting in two lacerations to scalp and a broken rib.

Miner was operating a Cat 992C loader and felt a burning sensation in right forearm. Visit to med. provider on 09-12-2011 diagnosed infected insect bite. Followed up on 09-14-2011 and admitted to hospital to begin treatment of Staph infected insect bite.

Miner was outside on operator platform of Caterpillar 990G loader positioning himself to wipe down the front windshield. Miner lost grip on handhold while positioning himself causing him to slip and slide down the front fender approx 15 feet to ground. This resulted in a fracture of the right heel/ankle.

Miner was walking on a loaded barge to obtain loaded freeboard measurements. Barge was fitted with a vertical rail on the deck. When stepping over the rail to the outside unloaded portion, miner put his foot on a rock which was on the outside of the rail. He rolled his ankle resulting in a acute right ankle sprain.

Miner(haul Truck driver)was sitting in cab of haul truck waiting to be loaded when he started having trouble breathing and was having chest pains. He contacted loader operator who contacted supervisory. 911 was called and miner was transported to local hospital. Miner suffered non-fatal heart attack. There was no record of previous heart issues.

Miner quality control lab manager was carrying a 6 gallon bucket of water across lab to dump out. He sat it down to obtain a better grip. Upon lifting, he felt a ""pop"" in his back. This resulted in a acute low back strain treated with meds and physical therapy.

Miner was attempting to remove a bearing cup from a wheel hub using a hammer and a piece of 3/4"" keystock. Upon striking the keystock, a piece of metal flew off and imbedded in miner's bicep requireing outpatient surgery for removal.

Miner operating a wheel loader dropped front tire into a mud filled depression causing machine to bounce.This resulted in a low back and neck strain. Was first notified of incident on 11-8-2010. Originally treated with Ibuprofen and muscle relaxer. After further diagnostics, it was determined on 12-10-2010 that physical therapy was needed.

Miner was helping remove a bucket pin from an excavator by holding a drive pin which was being struck by a hammer. Upon striking the pin, the bucket pin unexpectedly flew out. The momentum of the driving pin caused miners hand to become caught between head of driving pin and bucket. This required 9 stitches in middle finger and 5 stitches in index finger.

Absorption of radiations, caustics, toxic and noxious substances

Miner greaser was removing a plug from a hydraulic tank. The ""ecology"" check valve failed causing miner to be splashed by hydraulic fluid. Some of the fluid splashed behind his safety glasses with a small amount entering his eyes. After receiving medical attention, he received a prescription for antibiotic eyedrops.

Over-exertion in lifting objects

Miner was on empty barge at loading ramp securing the winch wire to a bow kevel. Barge was fitted with a high combing requiring him to lift the cable over the combing. This resulted in an acute right shoulder strain.

Struck by falling object

Miner was positioned on scaffold board cutting loose old steel angle iron with torch. Angle unexpectedly broke loose, fell and bounced off scaffold board with the end striking him in the left shin. This required 7 stitches to repair.

Flash burns (electric)

Miner electrician was inspecting stabs on bucket can in Motor Control Center. When re-inserting bucket in to cabinet, an insulating barrier became dislodged. This allowed the bucket to come in contact with 480 volt bus bars creating an arc flash. This resulted in 1st degree burns to miner's face and 3rd degree burns to both hands and arms.

Miner was lifting and carrying a gas powered 2"" pump when he stepped the wrong way and felt pain in his right calf. This resulted in an acute right calf strain.

Miner was removing bolts from hydraulic filter cover. The ratchet slipped causing the miner to become off balance. Miner reached out to support himself with his left hand and felt a snap in his left wrist. This resulted in a fracture of the left radius.

Miner was helping to free lumpy material from a hopper chute by hitting the side of the hopper with a 4# hammer. He managed to get a finger between the hammer handle and the side of the hopper. This resulted in a right index finger contusion as well as a nondisplaced fracture of the distal phalanx.

Miner was re-installing drive belt guard and discovered a bent bracket. While attempting to straighten bracket with 4# hammer, miner's little finger became pinched between hammer handle and drive sheave. This resulted in a fracture to the distal phalanx of the right little finger.

Miner picked up a piece of steel I-beam approx 2ft long weighing about 100 lbs. He carried it for approx. 5 feet and set it down. At this point he felt pain in his lower back diagnosed as acute low back strain.

Miner was inserting a flat steel plate between a conveyor belt and a troughing idler to use as a base for repairing a mechanical splice. Miner was hitting plate with 4# hammer and struck left ring finger with hammer smashing between steel and hammer resulting in a contusion and tuft fracture.

Miner was climbing down the ladder of a parked haul truck. When he reached the bottom step and was setting his free foot on the ground, he placed his foot on a softball sized rock. When his full weight was transferred to the foot on the rock, his foot rolled causing an acute left ankle sprian.

Miner was loosening jam nuts on a jaw crusher using an 8# hammer and hammer wrench. When nut broke loose, miner attempted to slow the momentum of the hammer and felt a pull in his back. Miner promptly reported but did not feel he needed medical care. On 08-11-2008 he requested medical care for continued stiffness. This resulted in a diagnosis of an acute high back strain.

Miner was climbing the outside of an inclined square chute, using welded on angle iron as a hand and foot hold. Hand hold broke loose, causing miner to slide approximately 4 ft. Miner's arm contacted a sharp protruding piece of metal, resulting in a severe laceration to the right inner forearm.

Miner supervisor was out on a ledge directing a loader building a ramp. He turned and started walking and mis-stepped on uneven ground. The uneven ground caused him to lose his balance and fall. This resulted in a strained left arm and some bruising and contusions.

On 06-10-2008 Miner was climbing ladder back up into a front end loader when he slipped on one of the steps. He caught himself with his right hand before he fell. This caused a strain in his right hand first digit. Miner reported promptly but thought he would be OK. Miner requested treatment on 06-12-2008.

Struck by falling object

On 05-27-2008 miner was removing a beam clamp from an overhead location from a rusty piece of metal. Clamp drug loose some rust flakes and a small flake entered miner's left eye, behind his glasses. Miner requested medical evaluation on 05-28-2008, which resulted in a particle removal with a prescription.

Miner was stepping down off a trailer mounted water pump. Miner stepped on and slipped on a wet stone. Miner attempted to catch himself by grabbing at pump frame and in doing so severly sprained right ring finger. Miner initially did not want medical treatment, but requested treatment the next day on 05-16-2008 when swelling increased.

Miner was sitting in cab of haul truck being loaded with wheel loader. The load in the bucket was extremely wet and released from bucket as a large mass causing truck to jerk and rock excessively. This resulted in an acute right mid-back strain.

On 05-02-2008 miner was helping to lift a track link for a Caterpillar 5130 shovel, when he made a turn and felt pain in his back. He continued to work. On 05-05-2008 he requested health care. This resulted in a diagnosis of an acute thoracolumbar muscle strain.

On 04-22-2008 Miner mis-stepped climbing down the ladder to his haul truck, causing pain in his right foot. X-rays were taken but couldn't be read until 04-23-2008. Contracted health care provider not available until 04-24-2008. X-rays negative. Pain gone. Miner released from care and returned to work on 04-24-2008.

Miner was handling port steel cable on a barge located at a barge loading facility. Cable had settled behind a raised steel plate. When the switchboat tightned up the starboard cable, the port cable became unlodged and struck miner in the ankle, resulting in an acute tendon strain.

Miner was in the process of moving a 7/8 steel cable with the help of 2 other miners, when he tripped and fell over the cable he was attempting to move. This caused him to fall against a hand rail, which resulted in an acute left posterior rib contusion.

Miner was lifting a wheel bearing from a cleaning cart and moving to a table. Miner picked up bearing and turned and walked towards table. Miner felt a pull in his back when lifting the bearing to table height. This resulted in a lower back strain.

On 03-13-2008 Miner was climbing stairs. Miner mis-stepped top step and scraped left knee and shin. On 03-18-2008 Miner requested medical treatment due to significant swelling and bruising. Normal care provider not available. ER visit on 03-18-2008 and followed up with normal care provider on 03-19-2008. Diagnosed with lower leg contusion.

Miner was removing a plug from a gear box using a hex allen wrench. When miner applied pressure to wrench, a paint chip popped out and entered miner's eye. Diagnosis was foreign body in right eye with no penetration. Foreign body was flushed and miner received prescription antibiotic drops.

Miner was cleaning a catwalk and using a 4 lb. sledge hammer to dislodge rocks in floor plate bar grating. He was swinging hammer with right hand and picking up rocks with left hand. Left hand entered path of hammer and was struck. This resulted in laceration and fracture of left index finger. This also required sutures.

Struck by falling object

Miner was removing a cribbing block from the top of a stand used to support equipment in the shop. Block slid off top of stand and landed on miner's foot. This resulted in oblique fractures of the 2nd and 3rd metatarsals.

Miner was using a 4 pound hammer to adjust conveyor belt idlers to assist in the tracking of the belt. He swung the hammer at an idler frame and missed. He then attempted to stop the momentum of the hammer. Attempting to stop the hammer resulted in an acute wrist sprain.

Miner was operating a wheel loader. When he exited the loader cab he noticed pain in his lower right back. Can't pin pain down to any one single event. He was diagnosed with an acute low back strain.

Miner was handling a cable in a barge moving procedure when the cable became taught. Before he could release the wire it jerked him forward quickly. Miner felt pain in the top of his shoulder and between his shoulder blades. This was diagnosed as an acute right shoulder/scapuler muscle strain.

Supervisor miner was stepping down from catwalk platform to the top of a storage bin. The hatch lid hinge is secured to the top of the bin with welded on ears that protrude up approximately 2"" Miner stepped on one of the ears and twisted his ankle resulting in an acute right ankle sprain.

Miner was operating haul truck approaching a curve when another operator of a haul truck approaching curve from the opposite direction failed to maintain control of his truck and skidded sideways in front of vehicle striking the cab with left rear of truck bed entrapping original miner and causing a broken leg.

Miner was crawling under piece of equipment in shop to remove a drain pan. Miner knelt on a foreign object possibly causing the skin to be broken. Miner jerked in reaction to foreign object and twisted knee. Also possible infection and celulitis in response to foreign object.

Miner was operating a front shovel in a tight muck pile making movements sometimes jerky and harsh. Jerky movement of machine caused miner to bounce right rib area on seat armrest resulting in a contusion.

Miner was walking down steps at plant and slipped on a step. Caught self on next step and twisted knee resulting in a knee strain which required an immobilizer. This incident appears to have aggravated a documented pre-employment condition of this knee.

Miner was climbing down ladder on wheel loader to check hydraulic oil level. He thought he was on bottom rung of ladder but was one rung higher. This mis-step caused him to fall to the ground resulting in a fractured right wrist.

Miner was washing pad under a conveyor that had choked up. Another miner up on the conveyor threw off a softball sized rock and struck other miner on hard hat resulting in a contusion to the head.

Miner was standing on bed of flatbed truck unbinding a detached backhoe boom and stick section. The loosening of one of the chain binders caused the load to shift and roll hitting the miner and knocking him off the truck resulting in cracked rib and knee injury.

On 4-17-2007 Miner was cleaning out under a conveyor tailwheel with a shovel and a rake. Miner was pulling out material wth a rake and felt a ""pull"" in his back. He attempted to give ""pull"" a chance to heal. On 05-02-2007 Miner requested a Dr. appointment. This resulted in a diagnosis of an acute lumbosacral strain requiring physical therapy.

Miner was walking down catwalk to control room. Mis-stepped on a rock and twisted ankle resulting in acute right ankle sprain.

Miner was working near the edge of a crushing plant bluff in harness and lanyard tied off to an air compressor. Compressor was still attached to a pickup. Another miner moved the pickup and compressor pulling injured miner approx. 12 feet causing a wrist sprain and elbow contusion. All miners in quarry have since been retrained in workplace exam and tie-off procedures.

Tightning a bolt with an air impact wrench. Air exhaust blew dirt under safety glasses into left eye, causing an irritation that required a prescription.

Using a recessed ring handle with right middle finger to lift cover off top of transmission compartment. When cover came loose it swung free and door began falling into transmission compartment. This pinched right middle finger between ring and door resulting in a smashed finger mallet.

On 01-24-2007 Miner was climbing down ladder of haul truck at barge loading facility and lost 3-point contact with ladder causing him to lose balance and fall. Miner received first aid treatment for ankle sprain. On 05-04-2007 Miner complained of knee and back pain. Sent to Dr. on 05-08-2007 as precautionary follow up of initial accident. This resulted in Physical Therapy.

The two people holding the plate at the top lost their grip and the plate forced employee's right wrist into the chute frame and twisted. He has a 6 cm fracture to his right radius and has restrictions that do not allow him to work.

Employee was stacking large steel hinge pins from storage on the floor of the warehouse onto a two wheeled hand cart. He felt pain in his abdomen and groin area. Possible inguinal hernia on right side. Surgical consultation 12/30/05.

Fall from ladders

Employee was stepping from a conveyor to a ladder on the rear of a 6' x 20' screen deck trailer. His left foot slipped from the ladder rungs and he slid down the ladder pulling muscles in his left calf and severely spraining his ankle. Further diagnostics revealed two possible small fractures to foot and ankle.

Struck by flying object

The employee was operating the controls of a hydraulic jack from a location six feet from the object being rotated. The jack was placed against a notch cut into the top ring of a seven foot diameter cone crusher. The corner of the notch failed and a shard of metal (2""x1"") flew six feet and struck the employee in the upper lip. The laceration required five sutures. No lost time.

The employee was pulling a 7/8"" diameter wire from the drum of a winch used to position barges on a loading ramp. He contacted a burr on the adjacent stern wire with his left forearm as he pulled. The burr caused a laceration approx. 1"" long, requiring 5 sutures.

HIS LOWER BACK PAIN PROGRESSIVELY INCREASED UNTIL 6-30 -00, AND WE SENT HIM TO THE DR'S OFFICE FOR PAINRELIEF AND PHYSICAL THERAPY. HE MISSED THREE DAYS AND RETURNED TO FULL DUTY ON 6-28-00.
