Employee was driving 5997 on the north side. The truck the employee was driving went through berm onto lower level. Truck rested on the ledge.

Employee was using a 10# sledge hammer to break rocks. Employee felt pain/pop in shoulder on down swing of hammer.

Fall from machine

Driver climbed up the ladder of the truck to do a pre trip inspection, he bent over to check the oil & water and fell off the truck. He landed on his left side resulting in a hair line crack of his hip, scraped left elbow and hand. Contributing factors: It rained last night so there was moisture on the truck and on his boots from walking to the truck.

EE had been running a rubber tired loader #4382 and hurt his back while operating the loader. Don't know when injury occurred EE's last day at work was 10/14 then he was rained out/lack of work. Injury wasn't reported until 10/17 to Foreman.

Employee exited her truck (5997) and went to shut the oiler off at the top of the Gencor (#6303) and slipped and twisted her knee. Knee strain w/small tear.

EMPLOYEE WAS HELPING MECHANICS PUT ENGINE IN TRUCK AND PASSED OUT. SPECIFICALLY GUIDING ENGINE INTO TRUCK WITH HAND SIGNALS. HE ALSO SPRAINED HIS ANKLE WHEN HE FELL.

WAS STRIPPING DIRT OFF WINTERSET ROCK AND BACKED OFF EMBANKMENT BETWEEN 20-30' WITH DIRT SLIDE UNDER. OPERATOR'S ERROR. IT WASN'T REPORTED UNTIL 0620 ON NEXT DAY. FOREMAN PICKED HIM UP AT QUIT TING TIME AND WAS NOT TOLD ABOUT ACCIDENT.
