Over-exertion (Not Elsewhere Classified)

He was welding a patch over a hole in the chute. He had to set in an awkward position to do the repair. When he was done he went to get up and felt pain in his lower back.

Fall onto or against objects

Employees were engaged in cleaning spilled material from the zero to six conveyor. Material was hand shoveled onto the belt and then the conveyor was jogged and the process was repeated. During the 4th. time the process was performed the employee slipped and his foot fell into a self cleaning tail pulley. The employees sustained a fracture to his left fibula.

EE had just idled up the generator when he turned around and felt a sharp pain in his upper back. He did not fall, trip, or anything else he just turned around.

Over-exertion in lifting objects

Employee was picking up a heavy battery with his right arm, (the battery was stuck). His arm popped and crackled when he first lifted it out of the battery box. He said it didn't feel right and it hurt immediately.

No Accident report was turned in until 10-8-09 Injury came in on that day and wrote the following report. While changing hammers or lid liners, picked up a liner and felt a sharp pain in the left arm.

Struck by flying object

Another employee was working on the hammer mill. EE was sitting in the hammer mill shell waiting to change the hammers. One of the hammers was stuck so the other employee hit it with a hammer, and a small piece of metal broke off and hit EE in the nose/cheek area. He thought he was ok. The area where he was hit would not heal properly and felt like it had something in it.

EMPLOYEE WAS ON HIS LUNCH BREAK WHEN HE DECIDED TO GO FOR SOME WATER. HE STEPPED DOWN WRONG ON HIS RIGHT ANKLE & TWISTED IT.

EE WAS SHOVELING OUT UNDER 2"" BELT THE GUARD ON THE BELT HAD A ROUGH EDGE & HE HIT IT WITH HIS LEFT HAND. THIS CAUSED A BUT BETWEEN THE THUMB & THE INDEX FINGER. THIS TOOK 4 SUTURES

HAD SHAFT ON TAIL PULLY BREAK INTO AND WAS REMOVING IT SO WE COULD REPLACE IT. CUT OFF BOLT WITH A TORCH, REMOVED GLASSES WHEN HE DID A OF HIS RIGHT EYE.

EE WAS WORKING ON A NEW JAW HOUS WHEN A 8"" I-BEAM PULLED OUT OF PLACE. WHEN HE WAS PRYING ON IT WITH A BAR (SO HE COULD WELD IT BACK) HE FELT A PULLING IN THE RIGHT GROIN AREA.

STEPPED ON A PIECE OF METAL THAT CAME UP AND HITHIS RIGHT LEG ON THE SHIN, GOUGING IT.

Fall down stairs

WORKING ON THE GROUND ON A LADDER ON LOADER, FIXING LADDER. EE WAS HEATING STEP AND I WAS USING PRY BAR TO STRAIGHTEN RAIL WHEN PRY BAR SLIPPED AND I FELL BACKWARDS TO LEFT SIDEWAYS. NO FEELIN G IN ANKLE AND FOOT FOR LITTLE WHILE.

Fall to the walkway or working surface

AS DESCRIBED BY EMPLOYEE, HE WAS STEPPING UP ON THE LADDER WHEN IT GAVE WAY AND HE HAD A BUCKET OF GREASE IN HIS HANDS. IT WENT ALL OVER HIM; HE HURT HIS LEFT LEG AND RIGHT REAR.

HE SAID HE HURT HIS BACK USING A BAR AND PULLING ON IT AND HE PULLED A MUSCLE IN HIS LOWER BACK HE LOST NO TIME. ON 3-2 HE STATED THAT HE REHURT HIS BACK ON THE JOB, HE ALSO SAID IT HAPPENED WHEN HE WAS PUTTING HIS BOOTS ON AT 630 AMHE DIDN'T START WORK UNTIL 7AM.
