Struck against a moving object

While running bobcat brake locked up bending back left thumb on hand control.

Fall onto or against objects

EE was emptying overhead bid into stockpile truck when handle to bin broke. EE's momentum caused EE to fall forward hitting head on truck and right knee on ground.

Struck by falling object

Putting block under loader bucket in shop. Loader was turned off and bucket dropped down on block and bruised the top of right foot on employee. Told EE we would take EE to get it x-rayed. EE said would ice it tonight and let company know how it is in the morning.

Fall from machine

EE was standing on the bumper of the haul truck in the shop tightening the bolt that holds the corner of the hood, his wrench slipped off the bolt and he lost his balance and slipped off the bumper landing on the floor. He fractured his left tibia and a hand bone (Thumb) in left hand.

Employee was grinding on bed of truck and fell off 6 ft. ladder in shop. He landed on right foot breaking his leg below the knee.
