Employee was in the shop preparing new conveyor belt for conveyor & while pulling around on it he had hurt his back. On January 26, 2009 He was put on worker's compensation due to his medication which would not allow him to run any equipment. To this date there was no lost time.

Rock got caught at opening of jaw crusher. EE was using a pry bar to turn the rock. The bar kicked back and pinched his finger between the bar and the guard around the jaw.
