Driving water truck to reduce dust on site. Worker alleges exposure to spray made him ill.

Employee was team lifting a small portable generator and strained him self to point of causing abdominal pain.

Struck against a moving object

Operator was using a loader to clear a waste pile at our plant pond and loader tire sunk into softer ground and loader rolled over on side. Minor damage was caused individual was taken to company physican against his request and checked out.

Fall from machine

Dismounting truck when foot missed the step, EE fell to the ground landing on his right foot.

Changing screens - When he was shoving the screen in place, he punctured right hand woth wire. He went to the doctor and received a tetnus injection.

Bucket of excavator slowly came down while setting electric motor on platform and hit ee's back.

Fall onto or against objects

Mud on feet. Slipped on ladder when climbing down off of auger. Put hand out and cut it on jagged edge.
