Bing Materials · Fall to the walkway or working surface

Miner was washing out under a tail pulley at the gravel plant, was standing on some ice gave the water hose a yank and slipped on the ice and fell to the ground. Smacked right eye brow on ground. If ee wasn't wearing safety glasses it would have been worst. EE received a small scratch and a bruise on right brow. No medical attention was needed.

Employee was operating a Skidsteer loader without seat belt in use, hit a mound of material, was ejected off the seat and when EE rebounded EE landed on right testicle, which in turn caused a "Intratesticular Fracture and Hematoma".
