Checking drive belt on Bagging Plant and it pulled finger between belt and pulley. Crushed right pinkey finger.

Employee was unloading steel plates from pallet on forklift. one of the forks slipped and hit employee in right knee causing cut and swelling. Employee was taking to quick care and treated. Returned to work with light duty on 2-20-14. Resumed to full duty as of 2-26-14

Employee was welding when a piece of slag dripped off the metal end onto his foot. The slag then burned through his boot and onto his toe.

PARTS RUNNER DELIVERING PARTS TO MINE SITE SLIPPED AND FELL ON DOOR JAM GOING INTO PARTS ROOM. EMPLOYEE SUSTAINED A BACK STRAIN. EMPLOYEE RETURNED TO WORK THE NEXT DAY. EMPLOYEE WAS ON LITE DUTY RESTRICTIVE WORK ONLY. EMPLOYEE RELEASED TO FULL DUTY STATUS 2-3-2005.

WHILE TIGHTENING A BOLT ON A MOTOR BOX THE SOCKET SLIPPED AND LEFT HAND WAS SLAMMED IN BETWEEN BAR AND MOTOR BOX. INJURY WAS SUSTAINED TO LEFT OPINKY FINGER. TREATMENT WAS GIVEN AT MANAGED CARE FACILITY. THERE WAS A FRACTURE TO THE LEFT PINKY. EMPLOYEE RETURNED TO WORK 5/14/04.

OPERATOR WAS FILING THE SURGE PILE WHEN HE NOTICED THE LONG TOWARD FEED BELT HAD STOPED HE PROCEDED TO STOP THE LOADER AND JUMP FROM THE TOP STEP OF THE LOADER TO THE GROUND WHICH RESULTED IN HIS INJURY.

EMPLOYEE WAS PUTTING ON DRIVE BELT WHEN HIS HANDSLIPPED AND HIT A PIECE OF METAL WHICH INJURED THE THUMB OF HIS RIGHT HAND.

EE WAS CARRYING A BOTTLE OF WATER (5 GAL.) WHEN SHE MISSED A STEP ON THE STAIRS AND FELL RESULTING IN A BADLY SPRAINED LEFT ANKLE.
