Checking drive belt on Bagging Plant and it pulled finger between belt and pulley. Crushed right pinkey finger.

Standing with hand in doorjam of shipping container. Wind caught the door and slammed finger.

strained neck, shoulder and back while transitioning from kneeling to standing while under loader. Hit head on the loader with hard hat on.

Strained right shoulder while back dragging rocks with shovel off of conveyor belt.

Employee was unloading steel plates from pallet on forklift. one of the forks slipped and hit employee in right knee causing cut and swelling. Employee was taking to quick care and treated. Returned to work with light duty on 2-20-14. Resumed to full duty as of 2-26-14

Caught finger on clip on conveyor and ripped open finger.

EE was cleaning on top of the hopper and slipped.

Was stacking bags on the pallet, sand came out the holes from the bag and shot in his eye.

Employee was welding when a piece of slag dripped off the metal end onto his foot. The slag then burned through his boot and onto his toe.

Broke bone in right foot after a piece of steel fell on his right foot.

While repairing catwalk with cutting torch a piece of metal went into eye.

PARTS RUNNER DELIVERING PARTS TO MINE SITE SLIPPED AND FELL ON DOOR JAM GOING INTO PARTS ROOM. EMPLOYEE SUSTAINED A BACK STRAIN. EMPLOYEE RETURNED TO WORK THE NEXT DAY. EMPLOYEE WAS ON LITE DUTY RESTRICTIVE WORK ONLY. EMPLOYEE RELEASED TO FULL DUTY STATUS 2-3-2005.

WHILE TIGHTENING A BOLT ON A MOTOR BOX THE SOCKET SLIPPED AND LEFT HAND WAS SLAMMED IN BETWEEN BAR AND MOTOR BOX. INJURY WAS SUSTAINED TO LEFT OPINKY FINGER. TREATMENT WAS GIVEN AT MANAGED CARE FACILITY. THERE WAS A FRACTURE TO THE LEFT PINKY. EMPLOYEE RETURNED TO WORK 5/14/04.

WHILE DRILLING THE EMPLOYEE STATES THAT THE DOOR SHUT ON HIS LEFT HAND.

OPERATOR WAS FILING THE SURGE PILE WHEN HE NOTICED THE LONG TOWARD FEED BELT HAD STOPED HE PROCEDED TO STOP THE LOADER AND JUMP FROM THE TOP STEP OF THE LOADER TO THE GROUND WHICH RESULTED IN HIS INJURY.

EE SAID HE WAS CHECKING BELT ALIGNMENT WICH CAN BE SEEN FROM OFFICE HE THEN CLIMBED UP ON BARMACSAID HE LOST BALANCE GRABED DRIVE BELTS THAT FEED CRUSHER RAN HIS HAND AROUND SHAFT BREAKING 2 FINGERS HE IS NOT SUPPOSED TO BE ON CRUSHER HE DID NOT HAVE AN EXPLANATION WHY HE DID IT MY ASSESMENT SAFETY VIOLATION, CARLESSNESS, WORK QUALIFICATION

EMPLOYEE WAS PUTTING ON DRIVE BELT WHEN HIS HANDSLIPPED AND HIT A PIECE OF METAL WHICH INJURED THE THUMB OF HIS RIGHT HAND.

EE WAS CARRYING A BOTTLE OF WATER (5 GAL.) WHEN SHE MISSED A STEP ON THE STAIRS AND FELL RESULTING IN A BADLY SPRAINED LEFT ANKLE.
