Struck against stationary object

Employee has had elbow pain that is increasing after FAI. On 03/01/16 employee received an injection of cortisone in elbow to relieve pain. Event was caused by aggravation of the FAI due to the work environment.

Caught in, under or between running or meshing objects

The employee was cleaning the rotary vane valve unit on a bag house discharge. The employee placed his hand into the vane valve housing, at which time the unit activated, rotated, and caught the end of a finger on his left hand, causing a crushing avulsion.

Struck by flying object

Employee was using screwdriver as a leverage tool while working on an wheeled loader while applying pressure to the screwdriver, the shaft of the screwdriver broke, and a section of the broken shaft flew up and struck the employee in the face (lip)

Fall to the walkway or working surface

EE was using a fire hose to move the bird balls away from the work area so the Guzzler could remove the sludge from the pond bottom. He had been working from the horizontal spillway but had to reposition himself to a sloped section of the pond liner to do his task. While moving the hose he slipped and tried to break his fall with his R hand. Fx hand, but released with restrictions.

Employee walking from break room to Process Plant slipped on snow covered ice fell to ground on Left Side seen by Ambulance crew released taken to Urgent Care for X-Rays and seen by doctor. Negative on X-Rays released with restrictions employee did not report for next shift on the Friday the 28th. Employee said he did not sleep and was sore.

Right hand on cable while head was moving.

Employee was lifting samples over the tailgate of the truck. The weight of the samples and distance to the top of the tailgate resulted in straining the lower back.

EE was cutting zip ties from stake with a utility knife and cut his left hand between thumb and forefinger.

Employee was walking down an embankment and slipped twisting his ankle.

Fall from machine

Employee was using a Quad to put in claim posts. Going up a hill the frame of the Quad came up and rolled back. Employee rolled out of the way and had scrapes to back & arms.

EE RET'D FROM ON THE JOB ERRAND TO PICK UP SUPPLIES FOR THE COMPANY. GOT OUT OF HIS CAR, WENT INTO LAB, SAT DOWN, SUPERVISOR OBSERVED HE LOOKED VERY ILL & SUGGESTED THEY CALL 911 AND GET HIM TO HOSP. FIRE TRUCKS, AMBULANCE, POLICE ARRIVED WITHIN 10 MIN. EE TAKEN TO CARSON TAHOE HOSP. LATER CAREFLIGHTED TO WASHOE MED FOR SURGERY.

EMPLOYEE WAS PULLING A PUMP AT THE MERRILL-CROWE. THE PUMP SLIPPED CUTTING HIS PINKY FINGER ON HIS LEFT HAND. HE WAS TAKEN TO THE HOSPITAL. FINGER WAS SEVERED, AND TOOK 11 STITCHES

I walked out to my car to check tire size (to get studded tires before the big storm). Clear day, no rain or snow, wearing tennis shoes, tripped walking back in from car to office. No lost work time but did see workmens comp. doctor on 12/28.

EE WAS HELPING THE CRUSHER FORMAN INSTALL A NEW CIRCUIT BREAKER, THEY WERE INSTALLING THE LAST BOLT TO SECURE BREAKER IN PLACE IT CAME IN CONTACT WITH A LIVE WIRE CAUSING IT TO SHORT OUT

EMPLOYEE WAS TRYING TO STRAIGHTEN A 4""X 21'PIECE OF PIPE. HE WAS KNEELING ON THE GROUND & TOLD THE LOADER OPERATOR TO MOVE FORWARD, WHEN THE LOADER MOVED THE PIPE FELL OFF THE BLOCKS & STRUCK HIS KNEE.
