While cutting a piece of pipe with a cutting torch, pipe fell off injuring fingers between it and another piece of pipe.

Employee was operating a D10 dozer feeding a sand & gravel operation. A rock unseen by the operator rolled behind him and he backed onto it.

Employee was in the process of cleaning off a feeder screen. He pivoted to the left on his right leg and experienced a pop in his right knee.

Employee was assisting in the removal of a rock in the throat of the jaw crusher. The chain that was being used slipped off the rock and struck employees hand that was on the handrail fracturing the thumb on the rt. hand.

EMPLOYEE WAS WELDING IN BREEZY CONDITIONS WHEN A PIECE OF METAL OR DUST GOT INTO HIS EYE. THE EMPLOYEE WAS USING ALL PPE FOR THE JOB (WELDING HOOD AND SAFETY GLASSES) THE EE STARTED FEELING DISCOMFORT AND IRRITATION IN THE EYE.
