Laborer was cleaning up next to the screen. A football sized rock bounced out of the screen and across the catwalk falling on the back of the employee's hard hat. The employee suffered a glancing blow to his head. He is on restricted duty for one week.

Struck by flying object

Loader operator was assisting mechanic in the changing of bucket teeth. The mechanic was striking a loader tooth on the right side of the bucket on a 988 Cat loader to remove it and a small piece of metal broke lose. The operator was on the left side of the bucket and caught the piece of metal in his left bicep.

Fall to the walkway or working surface

Employee was cleaning rocks from the walkway on the primary screen. He bent down to pick-up a rock that was stuck in the walkway and put his hand on the handrail. The handrail had a broken weld that gave way and he fell through to the ground (approx. 5 ft.) striking his head and back on the ground.

Fall down stairs

EMPLOYEE CLAIMED THAT A STEP BROKE & HE FELL HITTING HIS BACK ON A METAL PORTION OF THE LANDING. THIS IS A DISPUTED CLAIM OF INJURY. HE STATED TO OTHER EMPLOYEES THAT HE HAD HURT HIS BACK LIFT ING A FIFTH WHEEL TRAILER HITCH INTO HIS PICKUP BED AT HOME. PRESENTLY SIIS & OUR INSURANCE COMPANY ARE DENYING HIS CLAIM OF INJURY DUE TO THESE STATEMENTS.

ELECTRICAL PANEL GROUNDED OUT 480 VOLT BUSE BAR CAUSING AN EXPLOSION. UPON INVESTIGATION IT WAS DETERMINED THAT A HINGE PIN HAD WORKED OUT CAUSING A CROSS GROUNDING SITUATION.

STRAIN BACK AND KNEE WHILE LIFTING ONE END OF A SPREADER BAR.

WAS LIFTING A PIECE OF STEEP STRAP OUT OF THE BACK OF A SERVICE TRUCK WHEN HE TURNED AND STRAINED HIS BACK. LOWER BACK. STRAP MAY HAVE WEIGHED 20-30 LBS.
