EE WAS DRILLING AND GOT HIT BY DUST AND ROCKS FROM EITHER BLOW-BY HOSE OR HIGH WINDS AT JOB SITE.

WHILE LIFTING MANTLE WITH CRANE, EE WAS TRYING TO STABALIZE IT WITH HIS HANDS, THE 6 TON MANTLE PINCHED HIS R HAND BETWEEN THE MANATLE & A BELT WHICH WAS SITTING NEXT TO IT.
TAKING A PIECE OF STEEL FROM THE STEEL BIN. HE KNEELED ON THE SIDE OF THE BIN, GRABBED THE STEEL & STEPPED BACK TO THE GROUND LANDING ON A 6" DIAMETER ROCK AND TWISTED RIGHT ANKLE.
WHILE CUTTING AND WELDING IN THE TANK OF WATER TRUCK, EE BECAME ILL AND HIS EYES WERE IRRATATEDBY THE FUMES GIVEN OFF.
EE GETTING DOWN OFF LADDER, WHEN HE SLIPPED AND FELL OFF THE BOTTOM STEP OF TRUCK 463. EE FELL APPROX 4-5' HITTING LEFT SHOULDER ON GROUND.
WHILE SERVICING A DRILL, THE WIND CAME UP AND BLEW PIECES OF DIRT AND SAND INTO HIS RT EYE. EE WAS WEARING SAFETY GLASSES AT TIME OF ACCIDENT. TRYED TO FLUSH EYE OUT ON SITE . NEXT DAY ON 6-25 -96. EE STILL COMPLAINED OF SOMETHING IN RT EYE. WAS SENT TO DR. WHERE A PIECE OF DIRT WAS REMOVED AND PRESCRIPTION MEDICINE GIVEN.
WHILE TRYING TO PLACE BAR UNDER SHOVEL IDLER, HE RAISED THE OTHER END WITH THE OVERHEAD CRANE, WHICH CAUSED THE END HE WAS WORKING ON TO SLIDE AND PINCH HIS MIDDLE FINGER BETWEEN THE BAR AND W OODEN BLOCK, CAUSING A LACERATION- 13 STITCHES.
WHILE WALKING ALONG MUCK FACE, EE TRIPPED AND FELL. HIS RIGHT LEG HIT SOME SHARP ROCKS CAUSING A LACERATION. LACERATION TOOK APPROX. 10-12 STITCHES.
WELDER WAS WELDIMG WHEN HIS L. EYE STARTED HURTING HIM AND SWELLING SHUT. EE HAD EYE FLUSHED OUT ON SITE. EE STILL FEL SOMETHING WAS WRONG, SENT TO HOSPITAL WHERE A PIECE OF SLAGE WAS REMOVED.
VERY WINDY DAY BLEW GRAVEL/SAND INTO EMPLOYEES RIGHT EAR WHILE EMPLOYEE WAS STANDING OUTSIDE ON BREAK.
DRIVER STATED THE HE WAS TRAVELLING DOWN HAUL ROAD IN SEVENTH GEAR. HE ATTEMPTED TO SLOW DOWN BY USING THE SERVICE BRAKES. THE BRAKES WOULDNT SLOW HIM SO HE THEN TRIED TO DOWN SHIFT. HE TRAVEL ED APPROX. 1 MILE TRYING TO SLOW THE TRUCK DOWN,UPON REACHING THE PAD AREA, HE TURNED TO THE LEFT AND ROLLED OVER 2 1/2 TIMES.
HEAVY WINDS OPERATOR OPENED LEFT DOOR OF BLADE WIND TOOK THE DOOR SLAMMING IT OPEN. EE'S ARM WHEN WITH DOOR TWISTING IT BACK AROUND THE DOOR HINGE.
WHILE SITTING IN THE TRUCK CAB WAITING TO BE LOADER, THE SHOVEL BUCKET STRUCK THE TRUCK BED JARRING THE DRIVER.
EMPLOYEE WAS DRIVING HAUL PAK WHEN HE BECAME NAUSEA, HAD CHEST PAINS, AND FELT DIZZY.
DRIVER WALKED OUT ON THE DECK AT CAT WATER TRUCK TO CHECK WATER LEVEL IN A TANK THAT HE WAS FILLING. HE SLIPPED ON SOME SNOW ICE AND FELL APRROX 7 OR 8 FEET TO THE GROUND.
DRIVER WAS HAULING A LOAD OF ORE DOWN GRADE. ROAD BECAME MUDDY & SLIPPERY. DRIVER LOST CONTROL AFTER GOING INTO DITCH WHEN THE RIGHT FRONT WHEELCAUGHT ON HIGHWALL. THE TRUCK TIPPED OVER ONTO T HE LEFT SIDE.

TRUCK OPERATOR WAS OPERATING TRUCK 706 [HALPAK 385C] WHEN HE WAS OVERCOME BY FUMES FROM THE EXHAUST SYSTEM.
EE JUMPED OUT OF VAN TO UNLOAD BAGS OF GARBAGE AND SPRAINED RIGHT ANKLE.
DRIVER WAS GOING DOWN A 6% GRADE TO FILL WATER TRUCK. WHEN SHE STEPPED ON THE BRAKES TO SLOW DOWN, THE BRAKES DID NOT SLOW THE VEHICLE DOWN. THETRUCK PICKED UP SPEED & WHEN SHE CAME UPN A 90 D EGREE TURN IN THE ROAD, THE TRUCK OVERATURNED CAUSING SOME BRUISING & SEVERAL LACERATIONS.
WHILE PERFORMING LUBRICATION DUTIES ON HYDRAULICSHOVEL, OILER CLIMBED DOWN OFF THE SIDE OF SHOVEL, SLIPPED AND FELL, INJURING THE LEFT ELBOW.
ON 10-4 WE BECAME AWARE OF THIS INJURY BEING AN INDUSTRIAL ACCIDENT OUR EMPLOYEE CLAIMS THAT HE HURT HIS MIDDLE BACK WHILE WORKING ON A BULK TRUCK SEPTEMBER 19 1995 THE AWKWARD POSITION THAT HE WAS IN CAUSED A STRAIN ON THE MUSCLES
ATTEMPTING TO CHECK FUEL LEVEL IN TANK, USING A CIGARETTE LIGHTER TO SEE. UPON REMOVING THE FUEL TANK CAP, THE VAPORS FLASHED CAUSING 2ND DEGREE BURNS ON THE FACE AND HANDS.
SUBJECT WAS KNEELING DOWN & WHEN HE STOOD UP SOMETHING HAPPENED TO THE LEFT KNEE POPPED OUT OF JOINT STRAINED LIGAMENTS
WHILE CUTTING OUT THE LINER OF THE CONE CRUSHER, THE LINER FELL OUT STRIKING THE VICTIM ON THE HEEL AREA OF THE LEFT FOOT. THE LINER WAS NOT CRIBBED OR SECURED AT THE TIME OF THE ACCIDENT.
WHILE RUSHING TO SHUT OFF AN ELECTRICAL SWITCH EE SLIPPED ON THE WET FLOOR & SLID INTO THE STAIRWAY SUPPORT BECAME STRIKING HIS LEFT KNEE
EMPLOYEE WAS JACKING UP THE RIGHT FRONT AXLE ON A 85 TON EUCLID HAUL TRUCK TO CHANGE THE RIGHT FRONT TIRE. THE TIRE WAS ALMOST OFF THE GROUND WHEN THE JACK SLIPPED OUT STRIKING EMPLOYEE IN THE RIGHT KNEE.
DRIVER OF HAUL TRUCK BACKED UP TO THE BERM ON THE CRUSHER STOCKPILE NOT KNOWING THAT THE WALL HAD BEEN UNDERCUT. THE WALL SLOUGHED CAUSING THE TRAILER TO SLIDE DOWN APPROX. 20'. THIS CAUSED TH E TRACTOR TO LIFT UP IN THE FRONT, THEN FALLING OVER ONTO THE RIGHT SIDE OF THE CAB. DRIVER WAS NOT INJURED, JUST SHAKEN UP.
EE WAS FLAGGING BLAST SITE. THE AREA WAS SNOW COVERED. THE EE CLAIMED SHE STEPPED INTO A HOLE AND TWISTED HER KNEE.
