Employee was shoveling, put the shovel in the dirt and pulled with right arm and pushed with the left arm and said his shoulder, ""Popped out.""

While shoveling slag into wheel borrow the Employee turned wrong with a shovel full of slag and felt pain.
Shoveling slag overrun out of tank into a wheelbarrow. Employee kept doing the shoveling for long enough that it overused the wrist/arm.
SLAG WAS BLOWING OFF OF THE CONVEYOR DUE TO HIGH WINDS. SOME OF THE SLAG BLEW INTO HIS EYE.
EMPLOYEE WAS PUTTING A 100# BAG ON THE SPOUT OF THE BAGGING MACHINE AND HE HIT HIS THUMB ON THE SPOUT OPENING WHERE THE SAND COMES OUT.
