Driver attempting to free trip on trailer. Driver standing on ladder with another ee hauling ladder. Trap came free throwing ee off balance and falling to ground flat on his back and back of head.

Employee was working in shop area of mine, grinding metal with a hand held grinder. Employee did not have eye protection, a piece of metal flew into his right eye.

Miner was unaware that the lid of VSI crusher was on a swivel & able to tilt. He stepped on it and it tilted. He caught himself with his left hand and injured his left shoulder.

Employee was preparing his load to be hauled & he was tarping the materials in the bed of his truck, when he slipped off the truck & caught himself with his folded right arm. Employee complained of pain in right shoulder. Deciding the pain must be a pinched nerve, he elected to seek medical assistance.
