Employee was working in shop area of mine, grinding metal with a hand held grinder. Employee did not have eye protection, a piece of metal flew into his right eye.

Miner was unaware that the lid of VSI crusher was on a swivel & able to tilt. He stepped on it and it tilted. He caught himself with his left hand and injured his left shoulder.

Employee was preparing his load to be hauled & he was tarping the materials in the bed of his truck, when he slipped off the truck & caught himself with his folded right arm. Employee complained of pain in right shoulder. Deciding the pain must be a pinched nerve, he elected to seek medical assistance.
