Struck (L) knee on electric motor while maintaining loader in shop. Pain and swelling in (L) knee.

While removing bearing, tail pulley housing shifted smashing/pinning EE's right hand between bearing housing frame and crusher frame.

While pulling welding leads cables off reel, a sharp piece of metal lacerated his left hand index finger.

Employee was removing large rocks from feeder grizzley on crusher when lower back injury occurred.

Struck by flying object

Employee was hammering wedges to tighten a screen. He said his safety glasses were tinted and scratched and removed them to see. After removing safety glasses, he struck screen wedge with hammer. A flake of rust flew into right eye. He tried to wash it out with eye wash; did not work. Sent to clinic for treatment. Off work 4 1/2 days.

Helping another employee lift trash barrels into loader bucket, he immediately felt mid and lower back pain.

Sitting in office chair, chair broke, dumping him on the floor. He hit head on wall and injured right shoulder and arm.
